Job Description: Position Summary Reports to the Chief Executive Officer and works as a key member of the organization’s executive leadership team. The Quality Director is responsible for operationalizing the Health Center’s Quality Program and the day-to-day operational functions of the Health Center. The Quality/Practice Director serves as the Quality Director and Risk Manager for the Health Center and works closely with the executive Leadership team, Chief Medical Officer, Primary Medical Leads, and Quality Committee of the Board. Essential Duties and Responsibilities Oversees and monitors the development and implementation of a Quality Improvement Program as well as a Compliance/Risk Program Review and evaluate patient health records to assess quality care measures Produce reports and analysis on identified quality indicators Oversee the annual submission of the HRSA UDS annual requirement by working collaboratively with the CHS leadership team and other staff Develop and lead cross functional teams when improvement projects are identified. Develop a schedule to present policies and procedures to the board for review and revision; reflect and report out changes as needed. Management of critical event processes including recording of events, facilitating root cause analysis and performance improvement and/or training activities necessary to minimize or prevent risk. Assists with staff training and the development of clinical competencies. Works collaboratively with practice managers and leadership on patient safety and infection control Oversight of Credentialing and Privileging Program that practice managers and QI staff implement. Collaborate with all staff in areas of Quality Improvement With the support of the CMO and Primary Care Lead MD and Board Committee Chair, facilitate and organize Quality report outs from Quality Committee meetings. Coordinates and leads a Compliance program. Report compliance activities to CEO, COO, QI/Compliance committee Consults and reports on quality indicators, audits, performance data, care model activities/changes and goals to practitioners, leadership, QI committees, and the CHS board. Involved in the investigation of unethical or improper conduct or business practice. Monitors corrective action and subsequent compliance. Works cooperatively with other leadership to identify and direct methods to reduce fraud and abuse such as conducting periodic audits and developing effective lines of communication for compliance issues. Work with executive leadership to develop vision and strategy for the CHS offices; provide oversight to ensure practice managers and clinic lead are operating in alignment with clinical policy and procedure as well as board goals for CHS. Knowledge, Skills, and Abilities Strong knowledge of and experience in process improvement and facilitation. Strong analytical, numerical and reasoning abilities, knowledge of health information technology in integrated primary care settings, including design and use of data to improve quality. Knowledge and experience in information privacy laws, access, release of information, and release of control technologies. Qualifications Education Bachelors’ Degree or equivalent experience; RN preferred Experience Minimum of 4 years progressive management experience in a health care setting.
