The Palliative Care team at CentraCare is hiring an Advance Practice Provider - NP, PA, or CNS
This Advance Practice Provider will join an existing multi-disciplinary team of 4 Advanced Practice Providers, 3 Physicians, 3 Registered Nurses, a Chaplain, and a Psychotherapist
Our Palliative Care team focuses on providing adults and children relief from the symptoms, pain and stress of a serious illness. The goal is to improve the quality of life for both the patient and the family. As our Medical Director suggests, Palliative Care can be looked at like a road trip, and as an_ Advance Practice Provider, you will be there to provide support along the way.

The Palliative Care Advance Practice Provider’s main function is to collaborate with patients and families with complex medical issues and health care providers in achieving optimal patient outcomes. Services include:

What is the schedule for this position?
EDUCATION
Graduate from an accredited school of nursing required. Masters degree with formal training as a nurse practitioner or clinical nurse specialist in the same specialty area of practice in which they are certified. Minimum of 500 supervised clinical hours in palliative care or related field preferred.
EXPERIENCE
Experience in Hospice or Palliative Medicine is most preferred, geriatric and oncology experience also preferred. ACHPN (Advanced Certified Hospice and Palliative Nurse) certification required within 2 years of start date. Experience in geriatrics or oncology. Strong communication skills. Interest in leading and completing projects. Effective interpersonal skills, leadership skills, and clinical expertise in the care of patients with a life-limiting illness.
SPECIAL REQUIREMENTS
Current licensure as a registered nurse in the State of Minnesota required.
Current licensure as an APRN (Advanced Practice Registered Nurse) in the State of Minnesota required.
Licensure or Certification as a nurse practitioner or clinical nurse specialist in the state of Minnesota required.
ANCC Board Certified as Clinical Nurse Specialist required.
Written agreement (prescribing agreement) with a physician based on standards established by the Minnesota Nurses Association and the Minnesota Medical Association in a Memorandum of Understanding that defines the delegated responsibilities related to the prescription of drugs and therapeutic devices.
Current American Heart Association (AHA) BLS Healthcare Provider card required prior to completion of unit orientation.
Successful completion of medical staff office credentialing must be completed prior to working without a preceptor. Successful completion of third-party payor credentialing must be completed prior to billing for services.
