Licensed as AMFT, ACSW, APCC, LMFT, LCSW, or LPCC required. (1) year direct service experience with emotionally disturbed adolescents in a healthcare setting required (student internship and/or practicum time may be substituted). Experience working in an inpatient acute setting preferred. How you will make a difference: Prepares a comprehensive written psychosocial evaluation and assessment for admitted clients. Participates in treatment team meetings and is responsible for a coordinated treatment plan for each client. Provides individual and family psychotherapy. Responsible for permanency planning including assessing client's readiness for discharge; coordinating and planning with treatment team; making appropriate referrals for placement, case management and aftercare services; and coordinating benefits with Business Office personnel. Prepares and submits client documentation in EMR according to specified timelines. Provides consultation as needed with families on issues regarding admission, discharge, and aftercare plans. Coordinates client intake, treatment, and discharge actions with county partners.
