REGISTERED NURSE
JOB DESCRIPTION
JOB SUMMARY:
The RN is a licensed professional who provides direct and indirect patient care services primarily delivered on a visiting and intermittent basis in the patients’ home and in accordance with policies and procedures of the Home Health Agency.
REPORTING RELATIONSHIPS:
The RN is directly responsible and reports at regular intervals to the Administrator, Director of Nurses or Assistant Director of Nurses.
EDUCATION:
Must be a graduate of an accredited School of Nursing and currently registered with the State Board of Nursing Examiners.
EXPERIENCE:
Currently licensed as an RN by the state. At least one (1) year of experience in the capacity of an RN within the last three years. Home Health experience preferred with a minimum of one (1) year institution experience. CPR required.
JOB DUTIES/RESPONSIBILITIES:
· Assesses the patient’s condition and nursing needs; sets goals, prescribes appropriate nursing actions to meet those goals and the physical, psychological, social and rehabilitation needs of the patient, including discharge planning.
· Formulates a written nursing Plan of Care promoting continuity of care.
· Coordinates and participates in direct and indirect patient care activities in accordance with the patient’s Plan of Care.
· Continuously evaluates the effectiveness of the patient’s Plan of Care and the delivery of nursing care.
· Documents all events which influence patient care and submits documentation according to agency standards.
· The role of the RN is understood by the nurse and is an integral part of the job performance.
· Performs other duties as deemed necessary.
PHYSICAL REQUIREMENTS:
Lifting up to 50 pounds, use of fingers and both hands required or compensated for by the use of acceptable prostheses, speaking, hear conversational voice 15-feet (one ear) hearing aide permitted, ability for rapid mental and muscular coordination simultaneously, near vision correctable at 13” to 16’ to Jaeger 1 to 4, far vision correctable in one eye to 20/20 and 20/40 in the other, operation of motor vehicle and therapy equipment, and ability to comprehend normal speech.
ENVIROMENTAL FACTORS:
Outside and inside, working closely with others, protracted or irregular hours of work.
SCOPE:
Patient and staff advocate. Maintains contact with other internal staff. Maintains contact with physicians, families of patients, and other concerned persons.
