Position Summary:

• The purpose of this position is to provide day-to-day health care related administrative functions, to provide direct wellness care over site to residents, according to service plans, standard policies and procedures and state and federal regulations. The nurse also assures timely identification of acute medical problems and contributes to the safety, comfort and wellbeing of residents. This employee will also work closely with other Chicagoland Methodist Senior Services' personnel.

ESSENTIAL JOB FUNCTION

Assists in the assessment and admission of new residents to ensure proper orders for diet, therapy, medication etc. are transcribed and communicated according to procedure.

Observes and evaluates residents medical, mental and emotional conditions. Reports and documents any significant changes to appropriate staff service planned appropriately. Informs physician and family members according to procedures.

Oversight of a philosophy of care that is focused on optimizing function of all residents through physical activity as well as care practices that assure the prevention of complications and the highest possible function and quality of life of the individual.

Works with Director and DON to ensure that residents are receiving appropriate care per service plan.

Communicates on a regular basis with the AL Director and DON, regarding observations and suggesting of problem areas, resident and team member activities, concerns, needs, etc.

Maintain accurate, current health and emergency medical information and records on each resident.

Maintains security of all medications within the facility. Is responsible for the oversight of medications that may involve preparing, setting-up and recording all medications according to physicians orders. Ensures the inventory of all medications and records as needed. Notifies the physician when resident medication will stop or prescriptions will expire. Reviews Medication Log for accuracy.

Ensures physician written and verbal orders are complete and promptly and accurately documented, communicated and service planned appropriately.

Participates in and signs off on the formation or updates of resident service plans when necessary.

Provides explanation of nursing care to resident and families according to procedure to ensure understanding of nursing care provided. Advises residents about medications and health questions.

Administers emergency care when necessary. Responds to emergency medical concerns of residents in AL. Contacts emergency medical services for assistance when required.

Keep abreast of changes in health care services by attending seminars/workshops/webinars/teleconferences as approved by the AL Director; share information with other staff.

Attends in-services as required by state and federal regulations.

Assists in the training process of new team members and reinforces training standards to maintain a high quality.

Provides explanation of procedures and assistance to staff to ensure on going understanding and training.

Maintain and promote good relations with physicians and hospitals in the nearby community.

Ensures safety and security of residents through proper use of equipment and safety devices and by following proper fire, safety and disaster procedures.

Observes and reports needed repair and service of equipment and supplies.

Participates in on-call rotation and duties.

Assists with developing and maintaining nursing policies and procedures.

Prepares reports and other documentation as required by the Assisted Living Director, government agencies, and insurance agencies. Disseminates information as appropriate and necessary. Assures items are routed/delivered in a timely fashion.

Performs general office tasks in a timely fashion. Creates, retains, and maintains resident records and other data requiring retention, periodic review and update.

Promotes and maintains good relations with residents, families and visitors.

May assist with resident activities and/or transportation to appointments.

Reports to work on time and as scheduled.

Complies with the nurse practice act.

Responds to requests and comments courteously offering an alternative or solution. Maintains complete confidentiality.

Response time to call lights requiring professional nursing should be immediate unless involved with the care of another resident.

Exemplifies company dress code. When working in Assisted living as a professional nurse this will include a nursing jacket (color and style will be provided by CMSS.

Assists in keeping resident rooms and living environment neat, comfortable and safe. Keeps office and supply rooms presentable, professional and well organized.

Addresses residents by their preferred name. Assures that residents look their best each and every day. Places residents dignity above their own comfort.

Performs other duties as assigned.

QUALIFICATIONS

This position requires a degree from an accredited school of nursing, must be licensed by the State of Illinois as a registered (RN) or practical nurse (LPN). Two years of experience in geriatric, restorative and/or rehabilitative nursing is preferred.

The person must be tactful and posses effective communication skills (read and write, English); have the ability to follow oral and written instructions; and be able to communicate effectively with a variety of people and personalities. The individual must be courteous and relate well to others, have genuine compassion for senior adults and the disabled, project a friendly character, support customer service and team concepts. Must be able to verbalize understanding of written documentation, instructions, general policies and procedures.

The individual must have the ability to work with semi-limited direction; take initiative and have the ability to plan and carry out responsibilities, organizing own schedule and duties in order to complete responsibilities. Must have empathy with and understanding of the needs and expectations of staff and customers; and pleasant telephone manner and the ability to deal with the public in a pleasant and professional manner. The ability to maintain confidentiality and good judgment is necessary.
