JOB PURPOSE:
The Substance Use Disorder Counselor (SUD-C) provides, SUD counseling, case management and rehabilitation services consistent with the client treatment plan. Services include substance use screening/assessment, treatment planning, education, individual and group counseling and administering drug tests. This position is part of a treatment team including therapists, psychiatrist and SUD counselors. The SUD-C provides targeted case management services, including assessing for client’s case management needs and linkage to community resources or higher levels of care.
The SUD-C’s are energetic self-starters who are able to function comfortably in a team environment or independently and relate well to co-workers and County representatives. This position requires flexibility and the ability to manage time and multiple tasks with supervision. The individual in this position must be able to assume a wide range of responsibilities, provide timely responses to requests and deadlines and work well under pressure.
ESSENTIAL JOB FUNCTIONS:
1. Provide SUD services to clients consistent with their treatment plan.
2. Provide case management services linking clients to relevant programs/service as needed including higher levels of care. Consult with client treatment team as directed by Program Manager, including therapist and psychiatrist as appropriate.
3. Conduct phone screenings, complete intake documents and ASAM assessments
4. Facilitate individual counseling sessions, SUD groups and/or education classes as assigned.
5. Administer drug tests and track results in client charts.
6. Perform clerical duties relevant to securing and maintaining County approval for client’s services such as faxing, filing, reminder calls, updating SBAT, data entry (LACPRS, PPSDS, DATAR), information tracking.
7. Attend off-site and weekend resource fairs and similar events as assigned.
8. Attend all required meetings, supervision, and case reviews as assigned.
9. Complete Service and Coordination Plans, progress notes, and other County-required documentation within 24 hours of service delivery in two electronic systems SAGE and EXYM; meet the timeliness standards of state and county government funding and other agency funding sources.
10. Work one evening per week until 9:00 pm as directed by Program Manager.
11. Meet Agency requirements regarding client service hours, consistently provide services in keeping with all programmatic and contractual requirements.
12. Work rotating Saturdays, on average of 1x/month as directed by Program Manager.
13. Provide assistance to other departments as needed.
14. All other duties as assigned by Supervisor.
