Description

 The goals of Healthy Families Delaware Program are to improve birth outcomes, reduce rates of child abuse, strengthen families, enhance child health and developmental outcomes, and promote family stability and economic self-sufficiency.

Team Leader

  • Home visiting program seeking a supervisor to provide weekly reflective supervision and professional development following the evidence-base home visiting Healthy Families America model. 
  • Supervise a team of staff with backgrounds in nursing, social work, education and human service. 
  • Implements program operations and ensure compliance with the program model, and agency standards and procedures.
  • Ensures that supervision of staff utilizes principles and concepts of the parallel process and reflective practice (modeling therapeutic relationships, change theory, self efficacy, client-centered, strength based, and solution focused approaches).
  • Provide weekly reflective supervision and professional development following the evidence-base home visiting Healthy Families America model. Supervise a team of staff with backgrounds in nursing, social work, education and human service.
  • Provides regular case conferences and team meetings. 
  • May participate in the preparation and monitoring of the budget and contract to ensure appropriate use of funds. Expend budget to ensure compliance with fidelity to the model and contract agreements.
  • Demonstrates effective connections with the external community
  • Prepares and maintains timely documentation and records in accordance with agency, state, accreditation, and other requirements, policies, and procedures. Evaluate and implement agency outcomes and interventions.
  • Maintains agency standards regarding confidentiality, quality, and productivity.
  • Participate on the Delaware Home Visiting Community Advisory Board and subcommittees as appropriate.

EDUCATION and/or EXPERIENCE

  • Bachelor’s degree in Social Work Early Childhood, or related field, required. 
  • Prior training in Healthy Families America Core Training and ASQ training, required. 
  • Experience supervising and motivating staff, required. 
  • Experience in health education and home visiting experience, required.
  • Precious experience providing services to culturally diverse communities and families, preferred. 
  • Knowledge of infant and child development, parent-child attachment, required. 
  • Knowledge of community resources.
  • Strong oral and written communication skills.
  • Ability to work independently and accurately.
  • Background in prevention services to the 0-3 age population, preferred.
  • Must have valid driver’s license, which meets the requirements of the agency’s insurance carrier. 
  • Must be flexible, with the ability to work some evenings. Travel required to visit families, and to attend trainings and meetings.
