Description

Children's Medical Center is a busy general pediatric office that has serviced our communities for over 27 years. We have 4 locations and offer great benefits including paid time off, health, dental, vision, life, and long and short-term disability insurance. We also offer 401K and profit sharing after one year of service and opportunities to grow within the organization. We are looking for a team member who can serve in a full-time Receptionist position. The Receptionist is a member of the Patient Centered Medical Home Team and provides administrative support to the patient and family and other care team members to ensure smooth patient workflow and quality patient care. Hours include evening and weekend shifts on a rotated basis

POSTION SUMMARY:

The Care Coordinator is a member of the Patient Centered Medical Home Team and is responsible for providing care coordination services to a group of complex, special needs pediatric patients and their families across the continuum of care.

The Care Coordinator assists the Provider with the communication, implementation, assessment, and evaluation of the plan of care in partnership with the family and other members of the health care team.

SPECIFIC DUTIES AND RESPONSIBILITIES:

Care Coordinator Responsibilities:

1. Assists patients through the healthcare system by acting as a patient advocate and navigator.

2. Assists providers in the development and documentation of care plans for individual patients.

3. Aides in the development of office work flows for PCMH, tracking processes, implementation of clinical practice guidelines, establishing goals, and work process/patient flow improvement.

 4. Coordinates with the Referral Coordinator to obtain insurance authorization for appointments, if necessary.

5. Facilitates health and disease patient education, including leading group office visits.

6. Facilitates PCMH team meetings and quality improvement initiatives.

7. Actively manages assigned panel of chronic care patients, including establishing relationships with patients and families, conducting pre-visit planning, promoting patient self-management of disease and behavior modification interventions.

8. Provides follow-up contact with patients to ensure compliance with recommendations, medications, x-rays, referrals, specialists and PCP visits, dieticians, etc.

9. Assesses barriers when patients are not meeting treatment goals, not following treatment care planning or have not kept important appointments.

10. Coordinates continuity of patient care with external healthcare organizations and facilities including the process of hospital admission and discharge and referrals from the primary care provider to the specialty provider.

11. Manages high risk patient care, including management of patients with multiple co-morbidities or high risk from readmission to a hospital setting, including keeping a registry of high risk patients in the EHR.

12. Follows up with patients who are due for comprehensive health assessments and preventative screenings for patients.

13. Oversees well care and chronic care reminders.

14. Promotes clear communication amongst the care team and treating providers by ensuring awareness regarding patient care plans.

15. Facilitates patient medication management based upon standing orders and protocols.

16. Participates on a team for data collections, health outcomes, reporting, clinical audits and programmatic evaluation related to PCMH certification.

1. Exemplifies excellent customer service, persuasion, verbal and written communication, conflict resolution with patients/parents/guardians, visitors, and other employees; shows courtesy, friendliness, helpfulness, and respect.

2. Consistently demonstrates respect for the capabilities, different cultures and/or personalities of internal and external customers.

3. Relates well, and works collaboratively as a team member with, all levels of staff in a professional manner.

4. Attends daily huddles, weekly and monthly staff meetings.

5. Consistently alters plans/routines when situation requires and continues to perform without projecting stress/frustration that would adversely affect the work environment.

6. Maintains and ensures patient privacy and confidentiality.

7. Takes the initiative to proactively assist other Care Coordinators without direct supervision and to resolve problems with other departments and co-workers.

8. Maintains open and effective communication with providers and employees to ensure quality patient care.

9. Performs other duties as assigned.

ESSENTIAL FUNCTIONS:

1. Ability to work at a computer terminal for extended periods of time on a daily basis.

2. Physical requirements for this position include: hearing, seeing, speaking, feeling, reaching, pinching, bending, stooping, standing, sitting, walking, computer keyboarding, 10-key, phone operation, and repetitive motions.

3. This position requires sitting for long periods of time. Walking and/or standing is required routinely. This position requires exerting up to 25 pounds of force in order to lift, carry, pull, or move objects.

Qualifications


Education: Minimum high school diploma or equivalent.

Certification/Licensure: Licensed/Certified Medical Assistant, LPN or RN

Experience: 2-3 years experience in the medical or health insurance field.
