**Essential Duties:******

* Work with a dynamic team of professionals including nurses and social workers in our foster care program for children with special medical needs.

* Provide intensive casework services and fieldwork for children and their birth and foster families with the goal of providing permanency as quickly as possible.

* With medical staff, team-train foster parents in specialized child care skills.

* Conduct intensive outreach to connect children and families with agency and community treatment resources.

* Maintain written case records.

* Work in partnership with RN assigned to cases; collaboration includes conducting home visits whenever possible.

* In-person collaboration is an essential function of the job.

* Perform other duties as assigned.

**Minimum Qualifications:**** **

* Bachelor's Degree in Social Work or related field required, Master's degree preferred.

* Foster care experience preferred;

* Bilingual (Spanish/English) preferred.

* Flexibility, and ability to work under pressure and meet deadlines;
