Additionally, the lifespan compensation range of this position is: $22/hour - $24.03/hour.

Learn more about the Job:

The Community Resource Specialist will identify any community member needing assistance, support and assess for services needed. This individual will provide case management by conducting an intake, identifying a program that best fits the need, providing a referral and following-up with client to ensure that services were initiated.

Job Responsibilities:

  • Meet and/or follow-up with anyone looking for resources in the community
  • Develop relationships with providers and community organizations
  • Participate in appropriate community meetings
  • Provide input on communication and outreach strategies
  • Utilize Referral platforms, such as Unite Us, One Degree, etc., to refer to community partners for services and accept referrals into Antelope Valley Resource Infusion (AVRI) Community Family Resource Center (CFRC)
  • Conduct outreach at provider sites and community-based organizations to recruit potential clients.
  • Provide presentations to community partners and at outreach events
  • Contact potential clients from outreach to encourage participation in the program and enroll them, if appropriate.
  • Conduct client intakes, including collecting key information in a sensitive and nonjudgmental manner.
  • Develop trust and rapport with clients and provides referrals as needed during the intake process.
  • Identify appropriate resources for clients and provides referrals as needed during the intake process.
  • Work closely with all program staff to provide a continuum of services.
  • Complete all needed paperwork for each client in a timely and organized manner, including clear and well written progress notes.
  • Update and shares community resources.
  • Adhere to all program guidelines, policies and protocols and actively works toward achieving the goals of the program.
  • Attend and participates in all staff meetings, outreach community meetings, and individual supervision.
  • Participate in periodic webinars and conference calls
  • Attend all other AVRI CFRC and recommended meetings and reflective supervision.
  • Carry out other duties as assigned.

Job Qualifications:

  • Bachelor's degree preferred in Social Work, Psychology, Human Services, Public Health, or a related field with at least one year of experience in community outreach, or at least two years direct experience in community outreach, including administering intakes.
  • Experience working with underserved communities.
  • Familiar with resources in the community
  • Must pass background check.
  • TB test clearance.
  • Maintain CA driver's license.
  • Must have automobile and proof of insurance.

Knowledge, Skills, and Abilities

  • Bilingual in Spanish, preferred.
  • Ability to function as a member of a multidisciplinary team.
  • Ability to assess families across a broad spectrum of areas, recognizing professional boundaries and the need for consultation from other disciplines.
  • Experience working with families from diverse age, cultural, and ethnic backgrounds.
  • Empathy, warmth, and the ability to establish ongoing supportive relationships with an ethnically diverse population.
  • Knowledgeable about local community resources and able to refer families to appropriate agencies.
  • Self-starter, with the ability to take initiative and require minimal supervision.
  • Ability to establish positive rapport with diverse populations, utilizing cross-cultural skills and knowledge of cultural and religious differences, attitudes, and practices.
  • Ability to maintain confidentiality.
  • Ability to develop relationship with providers and community organizations and provide input on communication and outreach strategies.
  • Skill in organizing and maintaining accurate records.
  • Proficiency in Word, PowerPoint, Outlook, and Excel.
  • Written and verbal communication and interpersonal skills preferred.
  • Ability to multi-task and effectively prioritize.
  • Supports and adheres to the values and mission statement established by the AVRI Steering Committee.

Physical Requirements and Working Conditions

  • Will work in an office setting, via a virtual platform, and/or in the community providing outreach
  • Working with overburdened, often high-risk clients of low economic and varied cultural backgrounds, living in high-risk areas
  • Office, and possible community settings
  • Work often performed under stressful and emotional conditions
  • This position requires lifting/carrying up to 25 lbs., stooping, bending, squatting, sitting on the floor, walking, running (may be needed)
