Project EPIC aims to increase access and engagement of youth and their families who have experienced trauma and who receive primary care at Martha Eliot Health Center (MEHC) or Children's Hospital Primary Care Center (CHPCC) through implementing an evidence based trauma focused intervention Trauma Systems Therapy (TST) and components of its adaptation for refugees and immigrants (TST-R).
The Patient Navigator will act as a cultural broker, partnering with Project EPIC staff in the delivery of culturally responsive services, work directly with youth and their families to engage in therapy to support successful outcomes, and will serve as a liaison between communities, families, youth, and services providers.
The Patient Navigator will:
Train in TST/TST-R, particularly cultural brokering components
Work closely with patients and behavioral health teams to deliver the TST/TST-R intervention
Work closely with behavioral health team to assess patients for referrals to resources. Act as a liaison with internal and community resources. May contact hospital and community resources to ensure access to services.
Assume responsibilities for outreach, navigation, and care coordination for Project EPIC eligible primary care patients
Support Project EPICs outreach efforts to engage and educate patients and community members in evidence-based mental health treatment and stigma reduction education
Co-facilitate skill based groups for youth
Participate in the development and cultural adaptation of additional TST-R resources for youth and families
Assist in trauma-informed training to clinicians and other providers in other child-serving service systems as needed
Help parents and caregivers identify how they may best support the mental health and well-being of their children. This may include understanding mental health diagnosis and treatment plans
Facilitate direct communication between program participants and service providers. This may include interpretation and translation
Attend TST-R team meetings and consult with TST-R team members to support the delivery of culturally responsive services
Share community and cultural knowledge and perspective to improve the effectiveness of services provided by Project EPIC and partners
Documents patient care and collects required data. Tracks outcomes and supports strategic planning processes. Develops and maintains data.
To qualify, you must have:
Interest in and knowledge about mental illness/mental health and its treatment
Fluency in English and Spanish
Comfortable working in diverse settings and across diverse ethnicities
In-depth knowledge and expertise in the culture and language(s) of the Caribbean region (e.g., Dominican Republic) or Central America preferred
Previous direct experience providing health education or patient navigation in a community or clinical setting is preferred
Previous experience working with culturally and linguistically diverse populations
