The Perinatal Care Coordinator functions as a resource and advocate for pregnant and post-partum women. Conducts maternal mental health screenings and systematically reviews screening data, provides appropriate responses for identified needs around social determinants of health, collaborates with all involved teams and partners to ensure that patient and family needs are met. Provides care coordination support for families experiencing difficulties getting needs met; ensures optimal patient coordination, excellent collaboration between teams, with a focus on family experience and outcomes.
Minimum Education
Associate's degree in social work, nursing, education, health sciences or related field is required (Required)
Bachelor's Degree Bachelor's degree in Social Work, Nursing, Education, Health Sciences or a related field is preferred (Preferred)
Minimum Work Experience
2 years' experience in a behavioral health or other health care setting as a member of a multidisciplinary team providing care coordination services to patients. (Required)
1 year experience working with pregnancy and/or post-partum patients, preferably providing care coordination services. (Preferred)
Required Skills/Knowledge
Excellent communication skills required.
Ability to work with diverse patient populations.
Ability to provide culturally appropriate trauma informed care and communication.
Knowledge of resources available to meet the diverse needs of families in Washington DC.
