Your Mission:

The Community Health Navigator Associate Director will play a crucial role for CASL in the South Side Health Equity Collaborative (SSHEC) by overseeing the development, implementation, and management of community health navigation program. This position is pivotal in leading a dedicated team, fostering strategic community partnerships, and delivering high-quality, culturally competent health navigation services to underserved populations. The Associate Director will work closely with healthcare providers, community organizations, and stakeholders to improve health outcomes and reduce health disparities in Chicago's South Side.

Your Impact:

As the Community Health Navigator Associate Director you will:

Key Responsibilities:

Program Management:

  • Develop and implement community health navigation programs and initiatives.
  • Oversee day-to-day operations of the health navigation team.
  • Monitor and evaluate program effectiveness, adjusting as necessary.

Team Leadership:

  • Recruit, train, and supervise a team of community health navigators.
  • Provide ongoing support, mentorship, and professional development opportunities for staff.
  • Conduct regular performance evaluations and provide constructive feedback.

Community Engagement:

  • Build and maintain relationships with community organizations, healthcare providers, and other stakeholders.
  • Facilitate community outreach and education efforts to promote health navigation services.
  • Represent the collaborative at community meetings, health fairs, and other events.

Data Management and Reporting:

  • Collect and analyze data related to program activities and outcomes.
  • Prepare regular reports for senior management, funders, and other stakeholders.
  • Ensure compliance with all relevant regulations and reporting requirements.

Strategic Planning:

  • Contribute to the development and implementation of the collaborative’s strategic goals and objectives.
  • Identify opportunities for program growth and improvement.
  • Stay informed about emerging trends and best practices in community health navigation.

Must Haves

To thrive in this role, you should have:

  • Education
    • Bachelor’s degree in public health, social work, healthcare administration, or a related field
  • Experience
    • At least 5 years of experience in community health, healthcare navigation, or a related field.
    • At least three (3) years of supervisory and administrative experience
    • Demonstrated experience in program management and team leadership.
    • Experience working with diverse and underserved communities.
    • Education, training and applicable years of experience in similar work setting will be considered in lieu of a degree.
  • Skills
    • Strong leadership and team management skills.
    • Excellent communication and interpersonal skills.
    • Proficiency in data analysis and reporting.
    • Knowledge of healthcare systems and community resources.
    • Ability to work collaboratively with a wide range of stakeholders.
    • Spoken and written knowledge of English required
  • Additional Must Haves
    • Ability to travel locally as needed.
    • Flexible schedule to accommodate community events and meetings.
    • Commitment to the mission of reducing health disparities and improving community health outcomes.
