We are looking for a Care Coordinator or two to join our team they must have a CNA and also has their delegation certification (or be able to obtain it within one week of hire) Help coordinate the care of our patients and employees. The ideal candidate will have experience in healthcare and be able to provide comprehensive care coordination services to ensure our patients and employees receive the best possible care and help. The Care Coordinator will be responsible for assessing patient needs, coordinating with other healthcare providers, developing treatment plans, and providing education and support to patients and their families.
We are looking for 2 care coordinators.
We will train the right candidate who has CNA experience. Prior management experience is helpful but not required. Must be able to lead other caregivers and cover a shift on short notice if there is a call off.
There is room for advancement as well. Some of the duties are as follows:
