Position Summary
The Choice Home Health Aide is responsible for providing loving care to our home bound patients. Key responsibilities include assistance with Activities of Daily Living (i.e. bathing, dressing, ambulation, etc.), light house-keeping and simple meal preparation. A Choice Home Health Aide will be responsible, timely, a good communicator with their RN Supervisor, willing to enter information in an electronic medical record (EMR), have reliable transportation and most of all be a compassionate observer of their patients who depend on their aides for early recognition of changes in condition. We love our Home Health Aides, as they are a key part of the care team.
As a Home Health Aide, you will:
Provide direct patient care to patients under direction of the RN and according to the Aide Plan of Care and to include personal hygiene, ambulation, exercise, safe transfers and other supportive tasks as assigned.
Appropriately report changes in the patient’s condition.
Complete appropriate documentation in a timely manner.
Practice accepted infection control principles.
Promote positive, supportive, respectful communication to patient/family and other employees.
Provide a clean, safe and comfortable environment which promotes respect for patient, privacy and property.
Provide skills necessary to assist the patient with proper nutrition and adequate fluid intake.
Promote the agency philosophy and administrative policies to ensure quality of care.

Current CNA/HHA & LTC certifications
Valid Driver’s License
Valid Auto Insurance
CPR Certification
