Our client in Stony Brook is looking for Licensed Practical Nurses (LPNs) to join their team at the Long Island State Veterans Home.
Title: Licensed Practical Nurse (LPN)
Location: Long Island State Veterans Home, Stony Brook, NY
Pay Rate: $42 - $45 per hour
(Temp to Hire)

Evening shift 3 pm-11 pm

Night shift 11 pm-7 am

Schedule:
-Thursday to Wednesday each week
-Rotating biweekly schedule, including every other weekend
-Commitment to work one major summer holiday (Independence Day or Labor Day)

Licensed Practical Nurse (LPN) - Stony Brook, NY
Responsibilities:
-Provide and ensure nursing care to residents within the framework of medical and nursing care plans.
-Monitor residents closely to identify responses to treatments, medications, and changes in health status.
-Administer treatments and medications to residents per medical and nursing care plans.
-Assist with residents' activities of daily living, direct care, and supervision of non-professional staff.
-Document daily in residents' progress notes and assist with required weekly, monthly, and annual documentation.
-Communicate changes and incidents to immediate supervisors and complete necessary reports.
-Maintain policies and procedures related to medication management, including controlled substances.
-Share pertinent information and changes impacting resident care with supervisors, nursing staff, and other departments.
-Address concerns or issues expressed by residents and families appropriately.
-Provide direct care consistent with the comprehensive assessment, resident requests, and care plan goals.
-Collaborate with other professionals to ensure timely and coordinated care.
-Participate in the Quality Assurance Program and relevant committees.
-Assist in orienting new employees and provide clinical in-service training to non-professional staff.
Required Qualifications:
-Valid NYS LPN License
Preferred Qualifications:
-One year of healthcare experience working with an adult or aging population in a clinical setting.
-One year of long-term care experience.
-Experience using Electronic Medical Records (EMR).
Additional Information:
-The Long Island State Veterans Home operates on a biweekly payroll system.
-Work schedules start on Thursday and end on Wednesday each week.
-Days off (pass days) for this position vary based on the biweekly schedule.
