ABOUT THE PROGRAM: Nursing Home Diversion

Nursing Home Diversion and Transition (NHDT) services in collaboration with the Department of Mental Health and Addiction Services (DMHAS) focuses on reducing inappropriate admissions to inpatient hospitals or nursing homes to individuals age eighteen (18) or older who have serious and persistent mental illness (SPMI), or co-occurring psychiatric and substance use disorders. Providing services to individuals who are at risk for placement in a nursing home or who are presently residing in nursing homes and are appropriate for community placements.

NHDT services shall be defined as a set of short-term person-centered, age-appropriate, recovery-oriented activities designed to assist individuals to move into community settings through accessing and utilizing mental health and addiction services, medical, social, educational, vocational, housing and other recovery support services essential to improving quality of life and sustaining recovery in the community. NHDT services shall include face to face contacts with eligible individuals in the individual’s community or treatment setting and collaboration with service providers as appropriate.

Duties & Responsibilities

  • Collaboration with the DMHAS and the Connecticut Department of Social Services (DSS) to provide NHDT services to individuals identified as eligible for such services based on treatment history or other eligibility criteria determined by the Department.
  • Complete all assessments and documentation required by DMHAS within program assigned timelines.
  • Triage of referrals to appropriate treatment services. Referrals shall address identified behavioral health needs and recovery supports, and shall promote integration into the community;
  • Brief care and case management services that, at a minimum include, assistance with housing, support, and treatment services.
  • Facilitate team meetings with all community providers, state agencies and the individual being served.
  • Collaborate with other health care providers to determine and facilitate the level, type and services required to assure the health and safety of the client by developing a comprehensive person centered recovery plan.
  • Maintains an understanding of behavioral health benefits and remains current on covered benefits, limitations, exclusions, and policies and procedures, in regards to services;
  • Cultivate and maintain positive and productive relationships that assist individuals to attend and participate in services and activities that support their recovery; 
  •  Cultivate and maintain positive and productive relationships with Local Mental Health Authorities (LMHA), nursing homes, hospitals and other providers of services and supports to individuals receiving NHDT services;
  • Review and assess individuals in hospital emergency rooms and inpatient units who are referred to nursing homes and all DMHAS clients referred from within the Local Mental Health Authority’s service system who may require nursing home level of care to determine the least restrictive setting appropriate to the client’s needs and facilitate access to appropriate services;
  • Make referrals to, and collaborate with, the Department’s Home and Community Based Services Mental Health Waiver program, or with the LMHA and other service providers for appropriate community services and supports to prevent institutionalization or facilitate a community placement from an institution as needed.
  • Collaborate with the Department of Social Services (DSS) to determine whether a nursing home placement is appropriate.
  • Collaborate with nursing homes where DSS has identified individuals for NHDT services, to include monthly meetings with nursing home staff to monitor and document each individual’s transition status as needed.
  • Develop, in collaboration with a NHDT Case Manager, a support plan based on the assessment to facilitates linkages between individuals identified for NHDT services with appropriate supports and services.
  • Provide assistance to individuals to increase their independence and assist with access to needed recovery supports. 
  • Attend and participate in staff meetings, trainings, case conference, supervision and other meetings as assigned by CHR and DMHAS.
  • May perform other assignments and duties, as area needs require.

QUALIFICATIONS:

Education: Registered Nurse, Bachelors level preferred

Experience: 3 years’ experience with Adults with Sever and Persistent Mental Illness and Co-Occurring disorders.

Licensure/Certification/Registration: Registered Nurse

COMPENSATION*:

$63,634/annually min. to $95,452/annually max., dependent upon experience. 

*Actual rates are determined at the time of offer and are based off of a strict calculation process, as well as adherence to our internal equity policy. Many of our positions offer program/location differentials and/or sign-on incentives. (Please note, for part-time positions this range would be pro-rated based on hours worked)
