CINQCARE Community Health Worker Supervisor Position Description
Overview The Community Health Worker Supervisor reports to the Healthy Starts for Buffalo Project Director with accountability for providing strategy, judgment, organization, and evidenced-based analysis to influence decisions, and directly to meet CINQCARE requirements. At CINQCARE, we don’t have patients or customers – we have Family Members. The CHW Supervisor will supervise a team of 6 Community Health Workers (CHWs) as they provide non-clinical participant outreach and engagement, screening and assessment, coordination of non-clinical services and supports, family and psychosocial support and health promotion coaching, and monitoring of participant wellbeing throughout the perinatal period. Job Responsibilities The Community Health Worker Supervisor will have the following responsibilities:
Supervise a team of 6 Community Health Workers from CINQCARE and Buffalo Urban League to fulfill non-clinical HS care coordination/case management with participants
Process prospective HS participant referrals and dispatch CHWs to conduct initial screening and enrollment
Participate in and ensure CHW participation in all required HS program trainings
Partner with the Project Director to timely enter completed HS screening/assessment forms into required HRSA and CINQCARE data repositories
Support RN-CM’s completion and update of participant-centered care plans
Monitor quality of CHW care coordination/case management activities with participants
Coordinate and monitor care plan implementation and triage changes in status
Respond to and support participant transitions of care as directed by RN-CM
Participate in daily huddles and weekly case conferences
Strengthen lines of referral across SDOH and other social service partners/providers
General Duties The Community Health Worker Supervisor will have the following duties:
Leadership : The Community Health Worker Supervisor will lead in defining and executing strategies and solutions to create business value, including building a team to design, develop, and execute those strategies and solutions to deliver desired outcomes.
Strategy : The Community Health Worker Supervisor will establish the business strategy and roadmap: (1) improve outcomes for CINQCARE Family Members; (2) enhance the efficacy of other CINQCARE. business divisions; and (3) develop and deliver external market opportunities for CINQCARE products and services. In establishing the business strategy, the Community Health Worker Supervisor will define and innovate sustainable revenue models to drive profitability of the Company.
Collaboration : The Community Health Worker Supervisor will ensure that capabilities form a cohesive offering, including by working closely with other business divisions to learn their needs, internalize their knowledge, and define solutions to achieve the business objectives of CINQCARE.
Knowledge : The Community Health Worker Supervisor will provide subject matter expertise in solutions, including determining and recommended approaches, solution deployment and performance evaluation.
Culture : The Community Health Worker Supervisor is accountable for creating a productive, collaborative, safe and inclusive work environment for the team and as part of the larger Company.
Qualifications The Community Health Worker Supervisor should have the following qualifications:
Education: Bachelor’s degree or equivalent in human services, public health, social work or similar field with a record of strong academic achievement;
Experience: At least 3 years’ care coordination experience, preferably in greater Buffalo, with at least one year supervisory experience;
Strong, practiced familiarity with Buffalo-area public assistance programs, social service providers, and community-based organizations
Experience with electronic medical records, health information exchanges, z-coding, and related systems and tools
Excellent written and verbal communication skills and experience communicating with and participating in interdisciplinary care teams
Comfort working in a diverse, fast-paced environment
Lived experience in the HS project area / target population strongly preferred

Location : Buffalo, New York Compensation : $65K - $69K
