Position Summary:
  • The employee will provide diverse support services to children and families through Child Welfare in two program areas: Independent Living and In Home support services. Independent Living services are provided to youth in out-of-home placement and those who have exited out-of-home care. Independent Living prepares youth starting at age 14 ½ for independence after leaving out of home care. Position also coordinates transfer of services for young adults, ages 18-21, who have exited foster care to receive transition services through the Regional Independent Living Coordinator. In addition, the Independent Living position supports the ongoing child welfare units by performing a variety of support activities.
  • This classification will also provide in-home safety services for families involved with Child Protective Services. In-home safety services are a supportive service for families in their homes to create a safer home environment for children/parents, develop safe parenting practices/strategies, strengthen family relations, and identify and connect families to community resources. The goal of in-home services is to prevent child removal from the home and to assist in the family reunification process for families with children placed out of the home. Work is completed in accordance with established procedures, a social work plan, or as directed by the supervisor.
Examples of Work Performed:
The following duties are typical for this position. These are not to be construed as exclusive or all-inclusive. Other duties may be required and assigned.
  • Develop and support an effective independent living program which includes providing youth with job search skills, home management skills, budgeting and money management, future care planning/benefits, apartment rental options, transportation options, and vocational opportunities.
  • Conduct pre-assessments to determine the needs of youth and post-assessments to measure the progress of youth.
  • Create individual service plans which include measurable goals and objectives for each client in cooperation with the Social Worker assigned.
  • Gather and document information for state mandated statistics, including outcomes of youth served in the program.
  • Develop relationships with foster home providers, educating them on the services the department provides and encouraging them to practice these skills with the youth placed in their homes.
  • Provide outreach and engagement for older youth to continue participating and benefitting from services provided.
  • Assist in the development, implementation and monitoring of comprehensive Protective Plans or Safety Plans in conjunction with Child Protective Services worker.
  • Meet frequently with families to provide and monitor the services that control the family's specific identified danger threats.
  • Provide observations to assist in evaluating safety and if needed, participate in the development of an updated Protective Plan or Safety Plan when there is a change in family or household condition.
  • Work closely with assigned social workers and participate in multidisciplinary and family team meetings to share progress, challenges, and offer recommendations.
  • Provide transportation to children and/or their family to scheduled meetings.
  • Maintain accurate and timely electronic case documentation and time study information for services provided.
Knowledge, Skills, and Abilities:
  • Adhere to professional boundaries, ethics, confidentiality and privacy practices, policies, procedures, and regulations.
  • Knowledge of functions and services provided by the Marathon County Social Services Department.
  • Knowledge of available community resources that support the goals of the independent living and in home safety services program.
  • Knowledge of adolescent development and positive techniques to use in interacting with this age group.
  • Ability to work independently, apply standards and criteria, and exercise judgment in accordance with established program procedures.
  • Skill in conducting interviews by listening, observing, gathering facts, and recording facts accurately.
  • Ability to work with customers in an empathetic manner, establish effective relationships with customers, and respond meaningfully to their needs.
  • Ability to establish and maintain effective working relationships with other agencies, parents, youth, schools, and co-workers.
  • Ability to organize and maintain information and keep data records and file documentation current and accurate.
  • Ability to maintain confidentiality and work with others in a non-judgmental manner.
  • Skill in effectively using a computer for data entry, word processing, internet research, e-mail, etc.
  • Ability to contribute to a positive work culture that fosters excellent customer service and teamwork.
Qualifications:
Bachelor's Degree in a Human Services related field, required.
Two years' experience in a social services or human services related field working directly with children and families, preferred.
NECESSARY SPECIAL QUALIFICATIONS: Possession of a driver's license and a driving record that meets County standards; OR ability to obtain reliable transportation.
