City Net Homeless Services is Hiring a Housing Support Case Manager - Community Outreach Specialist Near Chula Vista, CA
Position OverviewIn this role, you will provide case management and supportive services to clients transitioning out of street-level homelessness at a non-congregate interim housing site in Chula Vista. Your focus will be on connecting clients to sustainable housing solutions and supporting them in achieving their personal goals. You will work collaboratively with various stakeholders to ensure clients receive the necessary services, including medical treatment, mental health support, counseling, and permanent housing.
Work EnvironmentThis position operates at a low-threshold interim housing site, with occasional meetings in professional office environments and community spaces. You will report daily to the Chula Vista office and may be dispatched for fieldwork throughout the area.

Requirements
- 1 year of case management experience with target populations affected by homelessness, substance use disorders, chronic health issues, and domestic violence.
- Passion for ending homelessness and making a difference in the lives of vulnerable neighbors.
- Empathetic, self-motivated, and flexible personality.
- Experience in cultivating and maintaining professional relationships with stakeholders.
- Understanding of quality and timely data collection and reporting.
- Excellent professional boundaries and communication skills in English.
- Strong interpersonal, analytical, and time management skills.
- Ability to work independently and as part of a team.
- Proficiency in PCs and Microsoft Office Suite.
- Ability to manage multiple projects and produce valuable results with minimal supervision.
- Must be available for variable scheduling, including early mornings, late evenings, and weekends.
- Valid CA Driver’s License and ability to drive a company vehicle, if required.
- Experience and professional contacts in homelessness services in Sacramento and surrounding areas.
- Experience with confidential client records management.
- Bachelor's degree or higher in Social Work, Human Services, Sociology, Psychology, or a related field.
