Join a Dedicated Outreach Team at City Net
**About the Role**
As an Outreach Case Manager II at City Net, you will provide vital case management and supportive services to individuals and families facing street-level homelessness. By conducting street outreach, you will connect unsheltered homeless neighbors with sustainable housing solutions. Working closely with clients, you will assist them in achieving their housing goals by developing long-term housing plans and connecting them with essential services like medical health treatment, mental health support, and counseling.
**Responsibilities**

• Conduct street outreach to support unsheltered homeless individuals
• Collaborate with various partners such as nonprofits, law enforcement, and medical providers to ensure clients receive necessary assistance
• Assist clients with developing and implementing long-term housing plans
• Provide care coordination and service connections to support clients in obtaining and maintaining permanent housing

**Requirements**
To Excel In This Role, You Must

• Have a minimum of 1 year of professional social services case management experience
• Possess an upbeat passion for ending homelessness
• Demonstrate effective networking and relationship management skills
• Understand the importance of timely data collection and reporting
• Exhibit strong verbal and written communication skills
• Be proficient in PCs, Microsoft Office Suite, and iPhones
• Hold authorization to work in the United States

**Nice to Have**

• Professional contacts in homelessness services in Whittier, Los Angeles County, and surrounding areas
• Spanish fluency
• Experience in maintaining confidential client records
• Bachelor's degree or higher in Social Work, Human Services, Sociology, Psychology, or a related field
