Case Management Counselor – Council on Aging

Salary Range: $16.00 - $18.00 per hour

Part time - 17 hours per week

The City of Attleboro Council on Aging seeks a Case Management Counselor, who working under the general supervision of the Council on Aging Director and immediate supervision of the SHINE Regional Director, provides on-site and telephone counseling to elders who need assistance with accessing public benefits and health insurance.

Duties include: Counsels elders concerning their health insurance options. Assists elders in completing forms for public benefits such as Medicare, Food Stamps, housing applications, fuel assistance, etc. Completes client intake forms. Submits monthly reports. Performs related work as required.

Requires high school graduation and one year of experience in a clerical or human services related field; or any equivalent combination of education and experience. Understanding of health benefits and public benefits is required. Ability to effectively work with elders and their families. Requires excellent customer service skills. Ability to maintain strict confidentiality. Successful candidate must have a valid drivers’ license and a Shine Counselor Certification. Additionally, the candidate will be subject to a CORI investigation.
