City of San Antonio is Hiring an Asthma Community Health Worker Near San Antonio, TX

Job Summary

Under general direction, is responsible for entry level professional community health work within the SA Kids BREATHE (Building Relationships, Effective Asthma Teaching in Home Environments) Program. Serves as a liaison between health and social services and the community to facilitate access to services and improve the quality and cultural proficiency of service delivery and work to improve health equity. Conducts outreach to implement programs in the community that promote, maintain, and improve individual and community health. Working conditions are in a community-based environment and an office environment. Exercises no supervision.
Individual in this position will provide asthma education and case management services to children and their families in their homes, and work with the child's school nurse and physician.
The San Antonio Metro Health District (SAMHD) provides public health services to clients, which may include risk for exposure to and possible transmission of vaccine-preventable diseases. Employees in high-risk positions will be required to obtain relevant immunizations, per SAMHD Policy DM13.2 (Download PDF reader). Review and consideration will be given to requests for exemption from this policy.

Work Hours
8:00 a.m. - 4:30 p.m. Monday - Friday; flexible hours in early evenings and weekends to meet with working families, as routinely needed; work from home on average 3 days per week to exclude during training

Essential Job Functions

  • Makes home visits to families of children with asthma, conducts assessments to assist with development of case management plan.
  • Follows all rules and laws protecting Protected Health Information (PHI) associated with the Health Insurance Portability and Accountability Act (HIPAA) as well as other regulatory agencies.
  • Employs strategic community building and organizing techniques to engage key internal and community stakeholders around health issues and health equity, including grassroots community based organizations, health centers, schools, community leaders, and community members.
  • Interprets, translates, or provides cultural mediation related to health services or information for community members.
  • Provides feedback to public health providers regarding improving service accessibility or acceptability. Provides consistent, documented follow up of appropriate resource referrals.
  • Refers community members to needed health and social services.
  • Collects information from individuals about the general health of community members and conducts data entry to track progress of cases.
  • Maintain updated client records with plans, notes, appropriate forms, or related information.
  • Advocates for individual or community health needs with government agencies or health service providers and participates in case management conferences with team.
  • Attends community meetings or health fairs to understand community issues and build relationships with community members. Provides asthma education to children and their families using culturally appropriate terms and concepts.
  • Contacts clients in person, by phone, or in writing to ensure they have completed required or recommended actions.
  • Distributes flyers, brochures, or other informational or educational documents to inform members of a targeted community.
  • Extensive community interaction is performed by using direct and other various methods of communication conducting outreach and visiting the community including using office equipment to provide health and social services.
  • Performs related duties and fulfill responsibilities as required.

Job Requirements

  • High School Diploma or GED equivalent (recognized by the Texas Education Agency or a regional accrediting agency).
  • Two (2) years of experience working as a Community Health Worker (CHW), health educator, community outreach worker, or community organizer.
  • Valid Class "C" Texas Driver's License or the ability to obtain a valid Class "C" Texas Driver's License within thirty (30) days after becoming a resident of the State of Texas.
  • Any applicant selected for this position must have a background check completed with results satisfactory to the City of San Antonio and successfully complete the San Antonio's Cash-Handling Certification Training.

  • Ability to help children and parents understand a health condition and develop strategies to improve the child's health.
  • Ability to work flexible hours, including routine early evenings or weekends to meet with working families.
  • Bilingual in English and Spanish.

One year of full-time experience is defined as 30 or more hours worked per week for 12 months. One year of higher education is defined as 30 credit hours completed at an accredited college or university.
  • Applicants selected for employment with the City of San Antonio in this position must receive satisfactory results from pre-employment drug testing and background checks. If required for the position, a physical, motor vehicle record evaluation, and additional background checks may be conducted.

  • Ability to perform all the physical requirements of the position with
    or without accommodations.
