Under general direction, is responsible for entry level professional community health work within the SA Kids BREATHE (Building Relationships, Effective Asthma Teaching in Home Environments) Program. Serves as a liaison between health and social services and the community to facilitate access to services and improve the quality and cultural proficiency of service delivery and work to improve health equity. Conducts outreach to implement programs in the community that promote, maintain, and improve individual and community health. Working conditions are in a community-based environment and an office environment. Exercises no supervision.
Individual in this position will provide asthma education and case management services to children and their families in their homes, and work with the child's school nurse and physician.
The San Antonio Metro Health District (SAMHD) provides public health services to clients, which may include risk for exposure to and possible transmission of vaccine-preventable diseases. Employees in high-risk positions will be required to obtain relevant immunizations, per SAMHD Policy DM13.2 (Download PDF reader). Review and consideration will be given to requests for exemption from this policy.

8:00 a.m. - 4:30 p.m. Monday - Friday; flexible hours in early evenings and weekends to meet with working families, as routinely needed; work from home on average 3 days per week to exclude during training
