Job Summary

As a Wrap Care Coordinator at CK Family Services, you will facilitate the Wrap-around planning process to the family. The following list identifies skills necessary for effective Wraparound Care Coordination. This list of skills should be conceptualized as a tool kit for Care Coordinators. The list is divided into the four phases of Wraparound, including Phase 1: Engagement and team preparation, Phase 2: Planning, Phase 3: Implementation and Phase 4: Transition.

You will also be trained in family oriented, individual and/or group rehabilitative mental health services, skills training and development, medication training, parenting skills training, and case management services to clients of the agency per state specified standards and requirements.

GeneralResponsibilities

  • Provide targeted (intensive) Case Management services to families and documented within 2 business days. Facilitate the Wraparound planning process and all the task required within each phase will be billed by Intensive Case Management. ***For all details please refer to the Wrap Facilitator checklist.
  • Actively uses the Engagement and team Preparation skills as identified by the Wraparound program throughout all phases. Facilitator’s responsibility is to engage the family and various team members throughout the Wrap Around process.
  • The Wrap Facilitator is expected to communicate and operationalize the four key elements throughout the Wrap process: Determined by Family; Strengths Perspective; Driven by underlying needs; and Supported by an Effective Team;
  • Actively demonstrates the ability to facilitate and plan Child and Family Team Meetings, but also demonstrate the ability to handle and resolve conflict.
  • Obtain regular updates from the Legal Caregivers regarding the child’s permanency plan as applicable or family’s vision and progress towards the plan beginning from the initial Child and Family Team meeting.
  • Effectively oversees the plan of care and monitors the progress by following up with each family and team member in between CFTM
  • Schedule and facilitate all Child and Family Team Meetings (CFTM) every 30 days.
  • Schedule and facilitate Emergency CFTM within 72 hours of a crisis occurring.
  • Develop a Crisis Safety plan upon initial meeting with the family and continue to monitor and update as needed. In the first team meeting, the Crisis Safety plan should be reviewed or revised.
  • Each Plan of Care needs to be submitted within five business days after the meeting.
  • Monitor the day to day implementation of the plan of care and tracking progress on a weekly basis.
  • Develop a transition Plan of Care with the team when the child and family are moving towards the end of the Wrap process to ensure that the family’s needs will be met.
  • Complete all case records accurately and timely within two business days.
  • Document services provided with the understanding that all clinical notations are considered legal documents and can be used in a court of law. All documentation should be true and accurate and under no circumstance falsified in any manner.
  • Participate in Wrap around supervision support
  • Immediately contact your supervisor and file a report with DFPS if you suspect a consumer has been the victim of abuse, neglect, or exploitation.
  • Immediately contact your supervisor if you have been called to testify or to be involved in legal proceedings.
  • Maintain client confidentiality at all times and be familiar with all HIPP laws and regulations.
  • Maintain professional certification and/or licensure, as this applies.
  • Present yourself in a professional manner at all times with an understanding that you are also representing CK Family Services.
  • Facilitators need to be aware of the formal services within the agency as well as within the community.
  • Serve on task forces or work group as requested.
  • Provide consultation to vendor agencies with whom our agency contracts, as required.
  • Obey all city, state and federal laws, rules and regulations.
  • Perform duties without violation of any ethical standards, state or federal codes, or agency requirements.
  • Be familiar with the Policies and Procedures Manual, Employee Handbook, and Employee Expectations.
  • Report any critical incidents, including suspected child abuse, neglect, or exploitation, immediately to the Abuse and Neglect Hotline and Director.
  • Ensure compliance with all rules and regulations of State and Federal regulatory agencies DSHS including licensure rules and regulations and suspected fraudulent practices.
  • Capture all changes and/or modification to the plan of care and send it out to the family and team members within five business days.
  • Participate in clinical meetings and agency administrative meetings as required.
  • Provide transportation as required.
  • All phone calls, messages, emails must be responded to within I business day.
  • Open and process mail / e-mail daily in a timely manner.
  • Answer phone, collect messages and respond to requests timely and accurately.
  • Input data into computer accurately and timely.
  • Represent CK Family Services in the community by attending meetings as scheduled.
  • Maintain confidentiality as required by government law and regulations, as well as maintain compliance with all CK Family Services policies, procedures, and state and federal laws.
  • Perform all other duties and tasks as requested.

Qualifications


Qualifications

  • Must possess a minimum of a bachelor’s degree in rehab counseling, psychology, social work, or related field as specified in the Texas Administrative Code for QMHP qualifications.
  • General understanding of adult and child mental health disorders, treatment of disorders, and documentation of treatment provided.
  • Knowledge of confidentiality and ethical guidelines for practice.
  • Excellent organizational and follow-up skills.
  • Excellent interpersonal skills and demonstrated ability to effectively communicate orally and in writing with a variety of people; maintain confidentiality
  • Trained in working with individuals with Severe and Persistent Mental Illness, Children with Severe Emotional Disturbances, and delivery of services based on curriculum.
  • Focus of work is primarily with areas such as preparing and reading data and figures and visual inspection involving small details. Visual work is primarily close to the observer. Depth perception and field of vision (peripheral) are important but not as critical as the ability to distinguish small details and markings very near to the observer.
  • Ability to receive verbal instructions accurately with some background noise.
  • Ability to motivate people; ability to develop and exercise good supervisory skills.
  • Ability to be on-call as necessary and respond immediately to a cellular phone.
  • Ability to handle multiple tasks; self-starter; energetic; detail oriented, with excellent organizational skills.
  • Good character, personal attributes and mannerisms to be a good adult role model for clients and their families, and as a professional for case managers under supervision.
  • Ability work effectively under stress and remain flexible in changing situations.
  • Ability to concentrate on moderate to fine detail with some interruption; ability to attend to tasks and functions for more than 60 minutes at a time.
  • Ability to understand and relate to multiple ideas and the concepts/theories behind the ideas; ability to remember multiple verbal and written tasks/assignments over several days and occasionally up to several months.
  • Proficient knowledge of computer software necessary such as word processing, email, spreadsheets, and database is required.
  • Ability to participate in training and facilitate training as required.
  • Ability to relate to multiples ideas and the concepts/theories behind the ideas; ability to remember multiple verbal and written tasks/assignments over several days and occasionally up to several months.
  • Perform the physical demands of the position
    • Ability to use up to 100 pounds of force occasionally, up to 20 pounds of force frequently, and/or up to 10 pounds of force continuously to move objects and/or people;
    • Ability to step backwards, grasp, bend at the waist and knees, lower 30-50 pounds to the ground, be able to get up and down from the floor without assistance, exhibit full range of motion in both arms and legs, and be able to do a deep, reverse lunge (SAMA Satori Alternatives to Managing Aggression).
    • Ability to walk frequently; stand or sit for prolonged periods of time; squat occasionally, climb stairs infrequently, and kneel and twist as needed.
    • Sufficient good health to properly fulfill all role responsibilities. Employees shall not be permitted to work who have infectious diseases or skin lesions for the duration of the communicability.
    • Ability to perform moderately difficult manipulation such as writing, typing, foot and hand-eye coordination required.
