Job Title:Developmental Home Program (DHP) Licensing Worker

Program(s):Developmental Home Program – Central

State:Phoenix, Arizona

Population Served:Developmental Disabilities (Adults & Children)

Reports To:DHP Statewide Supervisor

FLSA Status:Exempt

SUMMARY

The Developmental Home Licensing WOrker assists the DHP Supervisor in the coordination of operations of the CDH/ADH(Child and Adult Developmental Homes) program. This position ensures timeliness of reports and intervenes in problem area and provides consistency and knowledge of Division of Developmental Disabilities (DDD) to ensure program integrity and compliance statewide. This position will also carry a caseload.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Responds to vendor calls in a timely manner.
  • Intervenes in client emergency calls when support specialists are not available.
  • Participates in after-hours on-call.
  • Oversees Client Funds statewide.
  • Collects demographic data on cases.
  • Performs administrative tasks in Focus as directed by the DHP Supervisor.
  • Develops DDD recruitment activities and facilitates Meet and Greet meetings.
  • Conducts research on similar agencies to ensure HRT meets or exceeds industry standards for reimbursement and support to families.
  • Develops social media content to recruit new DHP program families.
  • Conducts trainings for both members and families which may include travel throughout the state as needed.
  • Assists other staff members with client coverage during leave and emergency situations.
  • Provides support and training to other DHP team members as directed by the DHP Statewide Supervisor.
  • Completes on-going monitoring of assigned homes for Article 10 and 11 (Child and Adult Developmental Homes) and Article 58 Administration of Children, Youth, and Families (ACYF Family Foster Homes) Licensing Regulations to assist assigned homes to maintain compliance with Regulatory Rules. License Monitoring activities may include, but are not limited to:
    • home visits (licensing, monitoring, unannounced; 1-2 times per month)
    • corrective action plan authorship and oversight
    • inquiry completion
    • health and safety assessment
  • Renewal facilitation
    • Quick Connect Orientation (Office of Licensing, Certification and

Regulation- Department of Economic Security).

  • Acts as a Consumer Advocate, assuring that Article 9 is maintained for all providers in assigned HRT Child or Adult Developmental Homes and licensed foster homes with a Developmental Disabilities(DD) Certification.
  • Provides Article 9 training and maintains certification as an Article 9 instructor.
  • Advocates for Children with American Indian Tribal Affiliation placed in an HRT home to assure cultural competency, service plan completion and maximum emotional, social and intellectual development of the child.
  • Attends consumer/client related meetings or staffings on assigned cases to include:
    • ISP (Individual Service Plans)
    • IEP (Individual Education Plans)
    • Medication Reviews
    • Foster Care Review Board
    • Behavior Building Plan Presentations
    • Other Events as needed
  • Maintains effective, positive communication with Tribal, State, and other Agencies encountered during duties with HRT.
  • Communicates regularly with HRT Developmental Home Program Supervisor regarding Central Program activities, needs, issues, successes, etc.
  • Assists Program Supervisor with pre-placement, placement, and post-placement activities for Consumers of DES/DDD and Tribal Youth. Assist Program Supervisor with other Program Projects as assigned.
  • Authors Behavior Treatment Plans (BTP) and present to State Program Review Committee. Monitor implementation of the Plan with monthly contact via home, school or day site.
  • Collects, evaluates, and distributes required monthly documentation from CDH/ADH Providers in Central Arizona.
  • Establishes delivery system to State Staff and Tribal entities requiring monthly/quarterly documentation for individuals in placement in HRT homes including BTP data sheet, Progress Reports, Financial Records, Habilitation Outcomes data collection and Teaching Strategies.
  • Maintains electronic Case Management (Client) and Provider Family Licensing Files for assigned providers in Extended Reach.
  • Attends company related staff meetings, Team building events, Provider Recognition, and other special events as needed.

TRAVEL

  • 50% throughout the county assigned.

REQUIRED EDUCATION AND/OR EXPERIENCE

  • Bachelors’ Degree in Social Work, Psychology or Human Services or related field and five (5) years’ experience with developmental disabilities, child welfare services and/or social services required or ten (10) years’ experience with developmental disabilities if no degree achieved.
  • Ability to obtain and maintain a valid unrestricted Level One Fingerprint Clearance Card.
  • Must meet the requirements to be a safe transporter and driver.
  • Must possess a current Arizona State Driver’s License and have an acceptable driving record for the past three (3) years.
  • Must have personal vehicle liability insurance and vehicle registration as required.
  • Must be able to pass a Department of Economic Security (DES) Clearance with no substantiated reports.
  • Must obtain and maintain Article 9 instructor certification.
  • When required, the ability to pass a criminal background clearance check, drug screen, and if requested a physical exam.
  • Must obtain and maintain CPR and First Aid certification.
