Clatsop Behavioral Healthcare is Hiring a Crisis Systems Case Manager Near Astoria, OR

This position qualifies for a hire on bonus of $1000 with 1/2 paid with 1st check after hire and 1/2 paid with 1st check following 6-month probationary period.

Clatsop Behavioral Healthcare is seeking a full-time Crisis Systems Case Manager. This position will mainly work in the community, interfacing with clients at locations varying from Emergency Departments to correctional facilities to local emergency shelters. The target population is primarily clients experiencing a crisis and/or involved in court processes such as the aid and assist program, and also participate in general jail diversion processes.

The Crisis Systems Case Manager will work with clients who are at various levels in the progression of their case, e.g. on conditional release programs, and will also serve as a CBH contact for efforts related to diversion from incarceration. Furthermore, the Crisis Systems Case Manager will be involved in outreach efforts and follow-ups for crisis clients to support the goal of diversion from incarceration or entanglement in the legal and/or healthcare systems.

This position works 12 hour shifts alternating 3 and 4 week shifts

Essential Duties and Responsibilities:

  • Participates in ongoing case management, ongoing needs assessment, and support of their treatment plan
  • Participates in jail discharge planning as needed in support of conditional release requirements
  • Collaborates with natural and community supports
  • Provides services with a collaborative, solution-focused approach
  • Collaboratively manages a variable caseload of clients involved or at risk of involvement in legal system, e.g. clients with unresolved charges
  • Provides services in the community at various locations: jail, court, emergency shelters, CMHP building, Emergency Departments, etc.
  • Documents case updates for Crisis or other cases in Care Logic, and shares information with all parties concerned
  • Performs outreach and engagement as a preventative measure i.e. diversion from incarceration or from a Higher Level of Care (HLOC)
  • Meets with clients to assess needs and considers additional resource referrals both within and outside of CBH
  • Coordinates with CBH program administrator (e.g. Crisis Systems Program Manager, Crisis Systems Assistant Manager, Exceptional Needs Care Coordinator (ENCC), or other designated staff) to provide accurate updates on client engagement
  • Assists with referral and coordination of entry into diversion programs such as Jail Deflection, Mental Health Court, or Drug Treatment Court as needed.
  • Assists with referral and coordination of entry into other programs within CBH as needed, when not duplicating services
  • Additionally provides in-person community outreach to clients and connection to natural supports and support providers of clients by visiting them in their homes, workplaces, or in the community
  • Changes method and degree of treatment when indicated by clients’ reactions as outlined in the treatment plan
  • Discusses progress toward goals with clients, clients’ families, and other natural support providers as appropriate
  • Regular consultation with other team members to ensure client needs are being met
  • Encourages use of identified coping skills, facilitates legal skills or other skills training groups, teaches life skills, and other evidence-based services
  • Refers clients to other supportive services to supplement treatment and counseling
  • Documents all services provided to participants according to agency standards and timelines
  • Consults and coordinates services with appropriate community partners such as: Emergency Department Social Workers, Developmental Disabilities, Primary Care Physicians, Department of Human Services, Senior & Disability Services, Probation & Parole, Community Action, schools, families, and natural supports
  • Obtains Releases of Information to be able to consult and coordinate care with other community agencies
  • Maintains client confidentiality and adhere to ethical standards of the profession
  • Participates in Investigations, court hearings, and other legal proceedings as directed by Supervisor and/or subpoena and maintains the required documentation for the same
  • Performs other duties as assigned.

Education and/or Experience:

  • Bachelor’s Degree in a behavioral science field Preferred. Must have or ability to obtain within 6 months of hire the Qualified Mental Health Associate (QMHA) certificate. CADC preferred but not necessary.
  • Prior experience in the field of case management working with individuals in the area of mental health or substance use treatment. Experience working in corrections or the legal system preferred
  • Knowledge of the DSM 5 & ASAM Levels of Care
  • Knowledge of processes related to mental health in Oregon (e.g. Aid/assist, civil commitment, etc.)

Dependents covered on medical, dental, and vision at cost to employee. CBH offers 40 paid hours of paid time for elective training per year in addition to mandatory trainings assigned by supervisors. CBH also offers relocation assistance and supervision towards licensure. Employees are eligible for elective training reimbursement of ($1000/yr for unlicensed clinicians and $1500/yr for licensed clinicians.)

12 paid holidays and 6.77 hours PTO accumulated per pay period for years 1-3 with increases after year 3. PTO accumulation starts at hire and is available for use as accumulated. CBH also offers sabbatical of 176 hours after 10 years of service.

Job Type: Full-time

Pay: $24.80 - $32.35 per hour

Expected hours: 36 – 48 per week
