ADULT DAY SERVICE PROGRAM AIDE - FULL TIMESUMMARY OF RESPONSIBILITIES: Provides personal care and respite services to customers of Adult Day Service (ADS) according to the Individual Plan of Care under the supervision of the RN Manager. Also, will assist with planned activities and crafts. SPECIFIC RESPONSIBILITIES:
Will exemplify the, “Service with Heart” attitude that is expected of all employees.
Responsible to help with Activities of Daily Living Skills (ADL’s) to include: eating, bath rooming, transferring, ambulating, etc., as appropriate.
Responsible for assisting the ADS Activity Specialist with planned activities and crafts.
Responsible for providing appropriate and safe respite services including personal care and companionship as assigned by the appropriate professional. Assist customers to/from the bus.
Provide warm, hospitable atmosphere for customers where they feel comfortable attending and participating and are not afraid to seek out help when needed.
Responsible for acting as a liaison between the customer and the RN Manager. This will include the prompt reporting of customers at immediate risk or endangerment.
Responsible for accurately completing and submitting all required Agency documentation within the required time frames.
Responsible for seeing that the ADS area is prepared for the next day.
Responsible to follow the CACFP and other funding source guidelines to prepare, heat and serve food and beverages.
BASIC QUALIFICATIONS:
High School diploma or GED
Certified State Tested Nurse Aide (S.T.N.A.) as verified by Ohio Nurse Aide Registry OR
Graduate of an approved Home Care Aide Program, without a 24-month lapse in employment since completing program OR
One year of supervised institutional or community based direct service delivery in healthcare within the last three years OR
Nursing students who can verify the successful completion of the Fundamentals of Nursing Care.
