As a Nurse Navigator Surgical Specialist Care, you will be responsible for coordinating referrals from physicians and health care facilities for patients referred to the Colo-rectal Health Center, providing a significant level of patient education related to their illness and planned treatment. This PRN position is anticipated to work about 16 hours per week.

This position will provide you with opportunities to work with the cancer team tracking volumes of the local population.

Responsibilities:

  • Patient Coordination. Works closely with physicians in all specialty areas to coordinate patient's care plan.
  • Assesses patient's health status by completing s creening tool and conducting an interview with the patient, family or significant other either in person or via the phone. Differentiates normal and abnormal. Notifies attending physician of findings needing attention pre- or post-operatively.
  • Clinical Protocol Development.
  • Works with designated physicians to develop and maintain clinical protocols.
  • Patient Education. Provides pre- and post-operative patient education related to their illness and planned treatment. Works with marketing and outreach departments to educate referring physicians and facilities on available resources.
  • Clinical Improvement. Proactively develops and enhances processes with the goal of improving the clinical experience for referred patients and the referring physician.
  • Liaison. Communicates with referring physicians? offices as appropriate to the patient and physician's needs Identifies problems or potential problems and initiates appropriate actions.
  • Functions efficiently with computerized charting.
  • Follows system and department policies and procedures with special attention to attendance and punctuality, confidentiality, dress code and display of ID badge, and safety.
  • Completes mandatory education and training in order to maintain organization and department specific competencies and requirements. Maintains applicable certification/licensure.
  • Other duties as assigned

Education:

  • Graduate of an accredited school of nursing; BSN/MSN preferred.

Certifications:

  • Currently licensed as a Registered Nurse in the state of Ohio. Maintains current Basic Life Support (BLS).

Complexity of Work:

  • Must have knowledge of basic computer skills. Excellent oral and written communication skills.

Work Experience:

  • Two years experience in nursing as a health or patient educator or as a pharmaceutical rep.
