POSITION: Children’s Case Manager
DEPARTMENT: Education and Advocacy

Position Overview:Under the supervision of the Manager of Education and Advocacy, this children’s case manager will be responsible for supporting the families of Cleveland Sight Center’s Early Intervention (EI) clients as they transition out of EI services to preschool. Additionally, this position will provide case management services to a fluctuating caseload of preschool through school-age clients who are in grades PreK-3.
Essential functions:
  • Collaborates with Early Intervention staff to support the families of clients and to prepare parents as they navigate the transition planning process (may begin as early as the child being 2 years, 3 months of age). The CM will also attend Transition Planning Conferences (TPCs) with county EI teams.
  • Takes initiative to educate and consult with EI families on the transition and initial evaluation process, IEPs, related services, and parent and child rights, etc.
  • Responsible for a fluctuating case management case load for school-age children in grades PreK-3. Case managers gather the medical and educational histories and diagnoses of children with visual impairments, obtain the relevant documents to assess needs and coordinate services with families, medical providers, educational professionals, and other community agencies to meet the visual, developmental, and educational needs of the client.
  • Case managers are available to provide ongoing education, consultation, resources, and support to families on an individualized basis. They may participate in ETR, IEP, and other school-based meetings, (required to travel occasionally within the community) and provide general consultation to educational staff working with children with visual impairments.
  • Provide service coordination, make relevant referrals, and maintain appropriate communication with other agencies or schools serving the child and/or family.
  • At times perform in the role of an advocate for clients and demonstrate the ability to navigate contentious situations with professionalism and tact.
  • Disseminate informational and educational literature that addresses visual impairments, its impact on functional and academic skills and access to education, and offer recommendations for appropriate services and supports to assist the visually impaired client.
  • Present information relevant to special education, services, supports, and best practices for teaching and caring for children with visual impairments or blindness.
  • Occasionally participate in outreach opportunities and public education (i.e. hospitals, schools, doctor’s offices, parent groups, community groups, and resource/health fairs) through written materials and presentations throughout the city of Cleveland and surrounding areas.
  • Assist with recruitment and placement of students for agency programs (summer day programs, Cleveland Sight Center Preschool, and social/recreational events)
  • Support families in the process of obtaining or re-establishing eligibility and subsequent funds through the family support programs in each county’s board of DD systems during/after the age three transition.
  • Support department and agency events that may be occasionally outside traditional working hours, including but not limited to, annual teachers’ workshop, outreach opportunities, White Cane Walk, etc.
  • Collaborate interdepartmentally with colleagues and agency co-workers
  • Other duties as assigned
Knowledge, Skills and Abilities:
  • Excellent and effective communication skills (verbal and written).
  • Knowledge of community resources for families of children with disabilities.
  • Solid knowledge of IDEA and Section 504
  • Must possess the ability to perform well in both a collaborative, team environment and independently.
  • Superb organizational and time management skills.
  • Ability to assess needs and develop an action plan.

Preferred qualifications:
  • Education: Bachelor’s Degree in Special Education or Early Childhood Education.
  • Experience: Minimum of 3 years experience in the Early Intervention, Special Education, disability services field, and or social work field.
Education and Experience:
  • Bachelor’s Degree in Special Education, Education, School Counseling, Social Work or Public Health Field.
  • Three years of relevant work experience in the Early Intervention, Special Education, disability services and or social work field.
  • Experience working with families of diverse cultures and backgrounds
  • Experience coaching, teaching, and presenting/public speaking.
Equipment/Tools Used:
  • Able to utilize electronic case management system and other applications such as Outlook and Microsoft Office, and virtual platforms (Zoom, TEAMS, etc.) proficiently

While performing the duties of this job, the employee must be able to lift up to 20 pounds and have the ability to stand, sit, bend, lift, carry, push, pull, or otherwise move objects and type for long periods of time. Must have the capacity to travel in a car; ability to go out in the community to make home, school, and community visits to, with, or on behalf of clients. The employee is occasionally required to use hands to finger, handle, feel objects, tools, or controls; and reach with hands and arms above shoulders.
Once hired, the employee must have reliable transportation. Employee/ driver must have current and sufficient automobile insurance and an acceptable driving record.
