EMPLOYER INFORMATION: Clifford Beers is a mental health clinic serving children and families. Our mission is to provide integrated services addressing mental, physical, and social determinants in order to improve health, resiliency and quality of life for children, families and communities.
PURPOSE:
The Program Manager will provide leadership and oversight of the Project Longevity, Victim Services Support Network, Community Healing Support Team (CHST) and Victim Services Support Network (VSSN) in the Specialized Community Services Department. This position will also lead each program's collaboration and outreach with Project Longevity Partners, City of New Haven and Town of Hamden Leadership and other community stakeholders in order to attain program goals and to ensure quality delivery of services for clients and families. The Specialized Services Program Manager will be responsible for coordinating internal and external services and support for individuals, families, and communities impacted by community violence or tragedy in New Haven and Hamden.
KEY RESPONSIBILITIES:
The essential functions of this position, include, but are not limited to:
Ensuring completion of prescribed forms following standardized procedures and timelines for all program referrals.
Maintaining waitlists and utilization reports for specialized community service programs.
Acting as liaison with Project Longevity Program Director, Program Manager, Case Manager, Outreach Workers, law enforcement partners, and other community agencies that provide consultation regarding Project Longevity involved members.
Acting as liaison with CHST and VSSN Program Director, Program Manager, Case Manager, Outreach Workers, law enforcement partners, and other community agencies that provide consultation and support regarding CHST and VSSN cases. Assisting with support and feedback to, and consultation with, CHST and VSSN staff and City Officials.
Providing ongoing feedback and support to Specialized Community Services teams under your supervision.
Assisting Specialized Community Services team day-to-day and in crisis situations.
Coaching and teaching the Wraparound process to staff, in-house, and in the community.
Participating in and presence at Project Longevity Call-ins and other community meetings and events related to Project Longevity, CHST and VSSN.
Ensuring and maintaining development of a balanced Family Team Meeting.
Assisting in quality assurance review of all applicable documentation and records.
Providing direct care to adults, children and families as needed.
Supervising, coaching, establishing performance expectations of, and evaluating program staff.
Facilitating effective communication among program staff, schools, internal and external partners to ensure seamless service related to Project Longevity, CHST and VSSN.
Participating in community and state training activities to promote individual professional growth and development.
Coordinate case conferences with the referring Project Longevity contact.
Coordinate and organize monthly program utilization meetings with the Specialized Community Services team.
Coordinate and organize quarterly meetings with additional Project Longevity program partners to discuss any gaps in services.
Coordinate programs utilization and planning for internal partners at CBCCC.
Coordinate CHST and VSSN rapid response to community-wide traumatic incidents
Develop interventions CHST will implement to de-escalates traumatic experiences
Assisting with support and feedback to CHST and VSSN to address urgent family and community needs related to traumatic events
Work with CHST And VSSN Project Administrator and external partners to identify and schedule neighborhoods for Community Canvasses.
Attending funerals, vigils, facilitating community circles, and canvassing related to CHST and VSSN.
Obtain Wraparound Coach/Trainer Certification in training modules in order to uphold fidelity to the Wraparound model.
Training and managing the required Wraparound Trainings to ensure that staff attain Care Coordinator Certification as put forth by the state.
Performing other related duties as directed.
EDUCATION AND EXPERIENCE:
Degree in a behavioral health or criminal justice field preferred (Three years related experience)
Experience working in an in-home setting, criminal justice and behavioral health

This role may require travel throughout the State of Connecticut.
