Job Description: Meta Care Inc., a dynamic healthcare organization, is seeking a compassionate and dedicated Member Ambassador to join our team in Denver, CO. In this role, you will be responsible for assisting our members in navigating the healthcare system, coordinating and scheduling their medical appointments, and providing necessary benefit support. This position requires a proactive approach to member care, ensuring that members have access to the resources and services they need.
This position does not involve hands-on clinical care or heavy lifting. Instead, it emphasizes the coordination of scheduling for preventive care and support for disease and chronic care management. It is a unique role that is mission-driven, dedicated to aiding with those we are entrusted to care for.Responsibilities:
  • Conduct outreach to educate members on preventative care and wellness initiatives
  • Provide educational materials and resources to members to help them understand and utilize available healthcare services
3, Schedule and coordinate healthcare appointments, including medical, dental, and specialist visits.
  • Assist members with setting up and understanding remote monitoring devices (e.g., glucometers, blood pressure cuffs, medical alert systems)
  • Coordinate the delivery of device supplies and other necessary health-related resources
  • Evaluate home safety and arrange for home modifications or meal services as needed
  • Identify and coordinate community support services, such as transportation and home care, for members
  • Assist members in navigating the coordination of benefits, working closely with healthcare plan design vendors
  • Develop and execute outreach campaigns to communicate benefits and services to members
  • Participate in client meetings to share information about available services and to address any overlooked needs
  • Follow up with members post-hospital discharge and conduct on-site hospital visits for those who opt in
  • Collaborate with company pharmacists and social workers to provide additional support to members
  • Maintain confidentiality and adhere to PHI and HIPAA guidelines at all times
  • Present professionally and respectfully in all interactions with members and colleagues
  • Travel to member locations and events as required, travel reimbursement will be given
  • Perform other duties as assigned by the supervisor
Requirements:
  • Minimum of 3 years of experience, preferably in a healthcare coordination or support role
  • Excellent communication and interpersonal skills, with a strong ability to listen and provide emotional support
  • Strong organizational skills, attentionto detail, and the ability to effectively manage schedules and maintain accuraterecords
  • Humble, personable demeanor with a genuine desire to assist and support others
  • Ability to work independently as well as collaboratively with healthcare providers
  • Proficiencyin Microsoft Office products (Word, Excel, PowerPoint)
  • Thispositionwould suitable if you areretired Nurse or has done nursing care in their past career and who is not currently practicing with a state licensure buthas a passion for coordinating nursingcarethough Licensed Practice Nurse (LPN), Licensed Vocational Nurse (LVN) Certified Nursing Assistant(CNA)inactive licenses are acceptable
Compensation and Benefits:
  • Comprehensive benefits package including medical, dental, and vision insurance, retirement plans, and paid time off
  • Salary Range: $18.00 to $25.00 an hour
  • Meta Care Inc., will contribute 90% of Medical Health Benefits
  • 10 days PTO/Vacation
  • 7 Statutory Holidays (New Years, Memorial Day, July 4th, Labor Day, Thanksgiving Day, Day after Thanksgiving, and Christmas Day)
