CSO is looking for a Relief Integrated Care Nurse.?
This Nurse will provide medical care and treatment for clients, ensuring coordination of care and appropriate discharge planning.

This entry level position provides the opportunity to practice as a nurse under the leadership of a highly compassionate, understanding, and skilled nursing leadership team. As a Relief Integrated Care Nurse:
You Will:
Provide medical care and monitoring for program clients, including medication order fulfillment and administration
Contribute to the Comprehensive Assessment and care planning of engaged program clients, including evaluating for substance use and withdrawal
Assist in the evaluation of clients in need of emergency treatment services for risk management, safety planning, and possible hospitalization, providing crisis counseling and crisis prevention planning as needed
Contribute to the development, adaptation and execution of transition plans, including medication reconciliation process as appropriate
Provide in-service training for staff on common medical issues and appropriate treatment, intervention and support strategies as well as provider psycho-education
Conduct medication reviews and monitoring as needed
Complete necessary reports and paperwork in a timely manner
Attend in-service trainings on evidence-based best practices and other topics as appropriate
Your Background:
Must be licensed in Massachusetts as a Registered Nurse?
Valid driver's license and reliable transportation
