LPN Care Coordinator

Compensation: $65,000 a year

Our rapidly growing medical practice that provides residents in Skilled Nursing Facilities with top-of-the-line care is looking for an LPN Care Coordinator to be part of a post-acute care management group that serves nursing homes. Job Qualifications:

• Proven experience as an LPN.

• LPN Certificate/ Degree

• Knowledge of health and patient care regulations.

• Ability to work full-time

• Strong communication and interpersonal skills.

Responsibilities include but are not limited to:

? Review patient records to ensure that information is accurate.

? Submit Review Visit Logs (RVLs) to supervisor.

? Adhere to the operational guidelines for color coding of patients.

? Respond to HR, operational, and clinical correspondences.

? Submit clinical corrections to the supervisor.

? Attend all operational and educational pod meetings as scheduled by the pod leader and submit the post-educational survey following these meetings.

? Promptly communicate with Operations to resolve any IT issues encountered

? You will be cross-trained in other areas of care management

? System Used: Emsow, Pointclickcare, Google Gmail, Sigma/Matrix, Reliable/Visual
