The Community Health Worker/Peer Support Specialist for our Behavioral Health Home program provides community-based integrated physical and behavioral healthcare services to individuals who may have complex and chronic mental illness, substance use, and medical co-morbidity in a fast-paced, interdisciplinary team setting. This position will work with individuals served to enhance access to services and work to achieve goals for mental and physical wellness.

This is primarily a community-based position. Staff will have to opportunity to work remotely on occasion and will also be expected to be in the office at times as needed.

**Qualifications:**

**Education**

* Required:

High school diploma or equivalent

**Experience**

* Required:

Office management experience

State-certified peer support specialist or trained Community Health Worker

* Preferred:

Experience working with persons who are mentally ill in either a hospital or community setting

**Licenses & Certifications**

* Required:

Valid and unrestricted drivers license
