COAL COUNTY GENERAL HOSPITAL is Hiring a HIM Coder Near Coalgate, OK
- Ensures that records are coded within 24 hours of discharge, excluding weekends and holidays.
- Reviews medical record thoroughly to ascertain all diagnoses/procedures.
- Contacts responsible physician in a professional, tactful manner, if diagnosis is not available on medical record.
- Coder’s diagnoses and procedures on clinical summary agree with physician’s preference 100% of the time.
- Maintains confidentiality for the protected health record.
- Refers medical record to director if there is a question regarding the diagnoses/codes.
- Utilizes computerized coding/abstracting equipment.
- Codes all diagnoses/procedures in accordance to ICD-9-CM and ICD-10-CM coding principles and the Coding Manual.
- Logs diagnoses, procedures and other abstracting data on worksheet for abstractor to input into computer.
- Meets productivity standard of assigning codes to a minimum of three (3) medical records per hour on federally funded patients and six (6) medical records on non-federally funded patients.
- Meets quality standards of having 95% of principal diagnoses and procedures appropriately and/or correctly coded.
- Maintains 99% rate of information correctly abstracted.
- Maintains the number of DRG/coding changes below the 3% quarterly per PRO threshold.
- Reviews coding periodicals within seven (7) days of receipt.
- Updates the Coding Manual on an ongoing basis.
- Notifies director whenever work is more than 48 hours behind work deadline.
- Assists the director with state requirements and reports.
- Acts as a resource person to hospital staff for coding and may provide education regarding coding changes/issues.
- Must be familiar with all medical record and coding requirements, including pediatric and SNF.
- Maintains a good working relationship within the department, other departments, and medical staff.
- Supports and maintains a culture of safety and quality.
- Accepts additional assignments and completes them within designated timeframe.
- Performs performance improvement functions through data collection and documentation review.
- All other duties as assigned by immediate supervisor and/or Director.
- High School graduate or equivalent.
- AHIMA Certification or equivalent, preferred.
- Knowledge of diagnoses/procedures in accordance with ICD-9-CM and ICD-10-CM coding principles for both acute and skilled nursing facilities.
- Ability to work with physicians in a collaborative manner.
- Able to communicate effectively in English, both verbally and in writing.
- Additional languages preferred.
- Basic computer knowledge.
