Cohesive Healthcare Management and Consulting is Hiring an Intake Coordinator - Hospice/Home Health Near Shawnee, OK

Description

SUMMARY:

The Home Health and Hospice Referral Intake Coordinator will be responsible for the daily operations of all admission activities. This could include monitoring the referral phone and fax line, verification of benefits, prior authorization when needed, and managing the intake process for new patients requiring home health and hospice services. The Intake Coordinator will be responsible for distributing information for review by the clinical coordinator for acceptance.

Requirements

RESPONSIBILITIES:

  1. Patient Intake:
  • Monitoring the referral phone line and referral fax
  • Receive and process referrals from physicians, hospitals, and other healthcare providers
  • Collect and verify patient information, including medical history, insurance details, and physician orders Verification of benefits – Medicare, Managed
    Medicare, Medicaid, Commercial payors
  • Start pre-authorization if warranted
  • Review information received to determine appropriate location for referral
  • Contact designated Clinical Coordinator to send information to for review for acceptance or denial
  • Communicate effectively with referral source
  • If accepted, determine projected start date of service 
  • Coordinate with team to ensure a seamless transition of care.
  • Help referral source with any transportation needs
  • Report daily census of each location 
  • Maintain patient records in compliance with state and federal regulations.

2. Documentation and Compliance:

  • Ensure all required documentation is completed accurately and timely.
  • Maintain patient records in compliance with state and federal regulations.
  • Verify insurance coverage and obtain necessary authorizations for services.

3. Communication:

  • Serve as the secondary point of contact for patients and families during the intake process.
  • Provide information and answer questions regarding home health and hospice services.
  • Communicate effectively with patients, families, healthcare providers, and team members to facilitate care coordination.

4. Administrative Duties:

  • Maintain accurate and up-to-date patient intake records.
  • Maintain daily patient database with detailed referral information
  • Prepare and submit reports as required by management.
  • Participate in regular team meetings and contribute to quality improvement initiatives.
  • All other duties as assigned

Qualifications:

  • Educations: Associate's or Bachelor's degree in Nursing, Health Administration, or a related field preferred.
  • Experience: Minimum of 2 years of experience in a healthcare setting, preferably in home health or hospice care. Experience in patient intake or care coordination is required.

  • Strong organizational and multitasking abilities. 
  • Excellent communication and interpersonal skills.
  • Proficiency in medical terminology and healthcare documentation.
  • Familiarity with state and federal regulations related to home health and hospice care.
  • Proficient in using electronic health records (EHR) and other healthcare software.
