Coleman Health Services is Hiring an ACT Case Manager - Stark County Near Canton, OH

Coleman Health Services is a nationally recognized not-for-profit provider of behavioral health and rehabilitation programs to foster recovery, build independence, and change destinies for individuals, families, and businesses in our community. We are seeking an experienced Case Manager to join a multidisciplinary team of providers who conduct assertive outreach in the community with individuals with severe and persistent mental health issues (SPMI) in Stark County, Ohio. Assertive Community Treatment Team (ACT) is an evidence-based program that engages with clients who are most at risk. The ACT team assists clients with accessing community-based resources, housing, employment, making appropriate referrals and assisting with maintaining scheduled appointments. Pay $20/hr.

ACT Case Manager Qualifications:

  • Must have BA/BS in mental health or related field and be passionate about working with our clients
  • Obtain an NPI and an Ohio Medicaid number
  • Maintain an excellent driving record
  • Must be able to work a flexible schedule including an after-hours on-call rotation. This position involves working in community settings, homes, apartments, etc.
  • Must be willing to complete and have favorable drug and alcohol screening if selected for the position
