Coleman Health Services is Hiring a Case Manager - Ravenna, OH Near Ravenna, OH

Coleman Health Services is seeking a Case Managers in Portage County, Ohio. Come join an energetic, fun, and supportive team that is part of a nationally recognized not-for-profit provider of behavioral health and rehabilitation programs. We are seeking Case Managers to provide care coordination and other case management services to individuals in Portage County who have mental health and substance use disorders.

Duties include:

  • Coordinate ongoing care with internal and external healthcare providers
  • Assist with symptom monitoring and coping skill development
  • Assist with making healthcare, service, and community resource referrals

Qualifications:

  • BA/BS in mental health/related field or three years' related experience
  • Have/obtain a NPI number and an Ohio Medicaid number
  • Have/maintain an excellent driving record

Preference given to the following candidates:

  • BA/BS in mental health/related field
  • Someone with a CDCA
  • Experience in case management
  • Experience working with adults in a community mental health setting
