Coleman Health Services is a nationally recognized not-for-profit provider of behavioral health and rehabilitation programs to foster recovery, build independence, and change destinies for individuals, families, and businesses in our community. We are seeking a Case Manager for our First Episode Psychosis and Action-Based Cognitive Remediation Program (FEP/ABCR) in Canton, OH. This is a hybrid position, encompassing general population clients in need of case management services and those individuals living with schizophrenia spectrum disorder.

The First Episode Psychosis and Action-Based Cognitive Remediation Case Manager provides clients with daily living activities, gaining greater independence, navigating systems, accessing, and acquiring services and provide crisis management in the community. The case manager will have a significant opportunity to intervene early and improve the lives of people affected by schizophrenia spectrum disorders with the hope of promoting recovery and preventing long-term disability. In collaboration with the Northeast Ohio Medical University, our case manager will coordinate with a team of behavioral health professionals to implement the 5 core FIRST treatment modules to improve the lives of our clients. This position also may be asked to take part in the Action-Based Cognitive Remediation program through NEOMED as well to provide our clients with interventions to further improve community functioning, everyday functioning, lower job stress, increased employment, and increases in cognitive abilities.

The FEP/ABCR Case Manager works within the limitations of the organization’s resources and in compliance with grants, contracts, and agency policies and procedures. The ideal candidate will take initiative and be able to build rapport quickly and effectively with highly vulnerable and exploited youth, young adults and mid-aged adults, comfortable navigating systems; reliable and efficient; committed to the self-determination of marginalized people; able to work collegially with colleagues and collaboratively with members of the FEP/ABCR team; steadfast in their goal of delivering case management services with impact. Must be willing to complete and have favorable results for criminal background check, drug/alcohol screening, and driving check if selected for the position
