Coleman Health Services, a nationallyrecognized not-for-profit provider of behavioral health services, is seeking anexperienced Case Manager to join our Crisis Services team as a Critical TimeInterventionist (CTI). Critical Time Intervention is a strategy of engagingpersons who are admitted to a psychiatric hospital for the purposes of optimaldischarge planning, transition back into the community, and engagement withoutpatient treatment services. CTIsassess individual's needs and link them to appropriate community-basedresources including housing, employment, ODJFS, and treatment services for aperiod of up to 6 months.

CaseManager (CTI) Qualifications:

·BA/BS in Human Service field

·Minimum of one year experience working with the SMD/AoD population

·Must have or willing to obtain a NPI and Ohio Medicaid number

·Must have an excellent driving record

·Must be willing to complete and have favorable results forcriminal background check, drug/alcohol screening, and driving check ifselected for the position
