Coleman Health Services is seeking a Case Manager forPortage County, Ohio. Come join an energetic, fun, and supportive team thatis part of a nationally recognized not-for-profit provider of behavioral healthand rehabilitation programs. We are seeking a Case Manager to provide carecoordination and other case management services to individuals in PortageCounty who have mental health and substance use disorders.

Duties include: 

  • Coordinate ongoing care with internal andexternal healthcare providers
  • Assist with symptom monitoring and coping skilldevelopment
  • Assist with making healthcare, service, andcommunity resource referrals

Qualifications: 

  • BA/BS in mental health/related field or threeyears' related experience
  • Have/obtain a NPI number and an Ohio Medicaidnumber
  • Have/maintain an excellent driving record

Preference given to the following candidates:

  • BA/BS in mental health/related field
  • Someone with a CDCA
  • Experience in case management
  • Experience working with adults in a communitymental health setting
