Due to ourproven record of success, we recently earned a nearly $4 million CertifiedCommunity Behavioral Health Clinic (CCBHC) grant to provide a comprehensiverange of mental health and substance use disorder services to vulnerableindividuals.

Coleman Health Services is seeking a full-timeGuardian to coordinate provision of legalguardianship services to adults in Stark County, Ohio. Individual will be responsible forcoordinating and implementing efforts to assist clients in maintaining optimumhealth care, safe living environments, and continued quality of life. Individual will work closely with othercommunity professionals to coordinate needed services to support theguardianship client and frequently be required to present in a court or legalsetting.

Musthave a Bachelor's degree related to social service or medical field. Knowledge of community resources, previouswork with probate court and guardianship services preferred. Knowledge of olderadult resources and community services a plus.
