Coleman Health Services is a nationallyrecognized not-for-profit provider of behavioral health and rehabilitationprograms to foster recovery, build independence, and change destinies forindividuals, families, and businesses in our community. We are seeking a JailLiaison, that will be responsible for coordinating and engaging clients inMH and/orSUD services in Canton, OH.

Responsibilitiesof the Jail Liaison Includes:

·Screen and evaluaterisks of clients and connect to services.

·Initiate dischargeplanning for clients to other area providers.

·Developrelationships with key personnel at local and state jail(s), courts, andprobation offices.

·Coordinate releaseplanning, including transportation, for all persons being released from jailwith a behavioral health and community provider(s).

·Provide advocacyand community outreach after release from jail within timeframeestablished via policy, as well as, tracking Performance Target Outcomes andWeekly Provider Reporting plus reporting to provider agencies on allinterviewed inmates daily.

·Communicate withservice providers about current legal status, treatment provided whileincarcerated and strengths and barriers to release.

Jail Liaison Qualifications:

·Candidates must have BA/BS in mental health or related field or1-2 yrs. of relevant experience with case coordination and case management withthe Adult Behavioral Health population.

·Must be eligible for anNPI number and an Ohio Medicaid number - we will help you!

·Must have and maintain excellent driving record.

·Must be willing to complete and have favorabledrug and alcohol screening if selected for the position.
