Colorado Behavioral Healthcare Council is Hiring a FACT LPN Injection Nurse Near Punta Gorda, FL
Coordinate injection visits for all FACT patient prescribed injectable medications.
Conduct home visits and administer injections to FACT patients.
Transport medications to patient homes.
Administer and document medication treatment.
Screen and monitor patients for medical problems and/or side effects.
Communicate and coordinate services with other medical providers.
Engage in health promotion, prevention, and education activities (i.e., medication education, overdose prevention, Narcan education, assess for high risk behaviors and attempt behavior change related to physical health.).
Educate team members on monitoring of psychiatric symptoms and medication side effects.
With patient agreement, develop strategies to maximize adherence to taking prescribed medication (e.g., behavioral tailoring, development of individual cues and reminders).
Document all activities in the Electronic Medical Record (EMR).
Associates, Bachelor’s, or Master’s degree in nursing.
Valid Florida LPN license.
Minimum of six (6) months working in medical/surgical, or psychiatric care setting.
Minimum of one (1) year experience working with adults with psychiatric disorders. Inpatient or residential experience is preferred.
Must maintain high standards of ethical and professional conduct, while adhering to agency policies and procedures.
