The SQI department encompasses a broad portfolio of contracts and grants, organized into three primary service lines (patient safety, population health, and community health) serving clients including federal and state governments. SQI includes quality improvement and public health experts who apply their skills to improve the delivery of health care across a variety of settings.
The employee for this position must reside within Clark County, Nevada.As part of the Systemwide Quality Improvement (SQI) department, the Southern Nevada Pathways Community Hub Director oversees the Southern Nevada Pathways Community Hub (Pathways Hub) operations and growth plan, including managing and directing the day-to-day work of Hub staff and subcontracted Care Coordination Agencies (CCAs), ensuring the Hub is operating in adherence with the Pathways Community Hub Institute certification requirements, ensuring the Hub is executing on contracts and grants that are funding its implementation with quality, on time and within budget, and leading business development activities to support growth and sustainability of the Pathways Hub.
Accountable to the Senior Director, Community Health, this position is primarily responsible for the successful leadership and completion of contract and grant deliverables, which contribute to the growth and sustainability of the Southern Nevada Pathways Hub.
  • This position will be responsible for supervising staff on the Pathways Hub team directly and in a matrix management role.
  • This position is responsible for building and executing on the growth and sustainability plan for Pathways Hub to ensure sustainability, including engaging Health Plans and State agencies and leading contracting activities with support from the Financial and Administrative Services Team.
  • This position is responsible for effectively representing the Hub with policy makers, health plans, State Medicaid, and other decision makers.
  • This position will implement a system to ensure ongoing quality improvement through monitoring performance data and using it to improve and refine Pathways Hub services, support the CCAs in operating in adherence with the model and identifying gaps in services to address health related social needs that can be shared with the Pathways Community Advisory council (PCAC), policy makers, and other key stakeholders.
Years of Experience in Related Field:8 years of experience in public health, healthcare, and/or social services.
5 years of management experience.
Education:MA/MS in a related field.
An equivalent combination of education and/or work experience in a related field may be substituted.
Specialized Knowledge, Skills, and Abilities and/or Competencies:
  • Comprehensive understanding of health and social care reform initiatives/environment.
  • Experience managing programs and large-scale projects, including project planning, budget creation and monitoring, and managing teams, subcontractors, and/or vendors.
  • Experience navigating contracting with health plans preferred.
  • Intermediate proficiency with Microsoft Office Suite (Outlook, Word, PowerPoint, Excel).
  • Familiarly with videoconference technology (e.g., Zoom).
  • Ability to organize and coordinate multiple simultaneous tasks in a team environment.
  • Excellent oral and written communication skills.
  • Experience with community engagement, health equity and social care frameworks and principles.
  • Experience with convening diverse multi-sector partners, with an emphasis on building equitable partnerships and developing collaborative solutions.
Leadership:
  • Conducts a variety of personnel management activities, including but not limited to recruitment, selection, on-boarding, training, evaluation, and separation, according to Comagine Health policy and practice and in collaboration with leadership and Human Resources.
  • Delivers, or ensures delivery, of initial orientation and training (including review of direct reports’ job descriptions), plus on-going training.
  • Ensures availability of procedures, technology, and other resources necessary to conduct work activities; provides subject matter expertise as applicable.
  • Provides timely, continuous recognition and feedback using objective, job-related data, and observations (including documented performance appraisals); timely initiates performance improvement opportunities in alignment with Comagine Health policy and practice.
  • Develops and monitors professional development goals and objectives; assigns work and delegates authority appropriate for established, job-related duties, responsibilities, and qualifications.
  • Develops, maintains, and enforces individual- and team-level performance expectations for deliverables; applies Comagine Health’s general standards of conduct.
Desired Outcomes:Guided by the organization’s values the Southern Nevada Pathways Community Hub Director contributes to achieving Comagine Health’s mission and vision through their leadership of the Pathways Hub and associated contracts and team. Typical duties and responsibilities include:
Growth
  • Leverages relationships with local partners and knowledge of community and public health needs to surface business development opportunities
  • Actively seeks opportunities to improve and expand programs and projects to leverage existing services and grow Comagine Health’s contract portfolio.
  • Supports business development activities (e.g., strategic planning, proposal response), including the development of a strategic plan to diversify funding sources for the Pathways Hub.
Financial Health
  • Monitors completion of timecards to ensure staff accuracy, timeliness, and compliance with related policies and procedures.
  • Works closely with SQI leadership to monitor financial performance against contract budgets and takes corrective action necessary in a timely manner to remain fiscally responsible for Comagine Health resources.
Operational Excellence
  • Sets the strategic vision to achieve Comagine Health contract goals and deliverables, including a focus on recruitment, data acquisition, design of effective interventions and methods of engagement.
  • Communicates the vision effectively to a team of staff and supports their implementation of that vision utilizing tracking mechanisms, regular team communication, and data review to ensure targets are met.
  • Coordinates with the state Directors, Community Boards, patient and family advisory councils, and other partners as needed to ensure the vision and work align with community needs.
  • Acts as key program spokesperson as needed. Convenes, delivers, and prepares panels and presentations for Senior Executives, Board of Directors, and external state and national audiences.
  • In close collaboration with SQI Senior Leadership, provides leadership, strategic direction, and oversight of project contracts (federal, state, and/or local initiatives). Ensures sufficient internal coordination of issues/topics related to assigned contracts so that there is adequate communication and contract deliverables, and client expectations are efficiently and effectively met.
  • Ensures project teams are meeting contract deliverables and evaluation measures utilizing a strategic approach, including coaching and mentoring of team members, supporting the creation of well-defined program plans, and consistently engaging local community partners and customers to ensure Comagine Health initiatives are aligned with and responsive to community needs.
  • Ensures that customers receive timely and appropriate contract deliverables with requested services. Initiates timely and appropriate communication, and other managerial interventions with internal staff and external partners to identify and resolve issues.
  • Uses the principles of QI internally to identify and implement better processes in partnership with the teams.
  • Develops, monitors, and reports on department goals, standards, and objectives, through collaboration with the Senior Director, Community Health or designee.
  • Ensures that the Senior Director, Community Health and/or other leaders are informed in a timely manner regarding significant operational issues, performance measures, complaints/grievances, compliments, quality management initiatives, staffing concerns, and risk management issues.
  • Maintains compliance with organizations policies and procedures, including but not limited to the strategic plan, organizational structure, confidentiality, safety, and complaint/grievance resolution.
  • Utilize customer database (CRM) for project monitoring, as appropriate.
  • Understands and adheres to Comagine Health’s Code of Business Conduct and Ethics, Corporate Compliance Program and related policies and procedures.
  • Participates in Comagine Health’s internal quality improvement activities as appropriate.
  • Performs related duties as required and assigned.
