AIM OF THE JOB

Under the supervision of the Managing Owner, the RN Care Manager is responsible for supervising comprehensive caregivers, and oversight of care for comprehensive clients. This includes, but is not limited to, ensuring comprehensive client assessments and reassessments are completed in a timely and compliant manner, provide skilled nursing visits including medication set up, conducting supervisory visits for comprehensive caregivers, reviewing care logs, participating in quality assurance activities, assessing caregiver capabilities, and training caregivers.

DIMENSIONS

The RN Care Manager:

· Performs home evaluations of comprehensive clients, assesses client needs, develops and documents client plan of care.

· Conducts weekly orientation and competency assessments for new caregivers as part of a training team

· Works with management Team to create and manage agency policies and procedures to ensure compliance with MN statutes

· Ensures quality of care throughout the company

· Is a back up for orientation and education of caregivers

· Provides skilled nursing visits including medication set up

· Oversees comprehensive Caregivers, to include day-to-day management and regular performance evaluations.

· Conducts regular on-site supervisory visits of comprehensive caregivers.

· Regularly re-evaluates comprehensive client plans of care to ensure client needs are met.

· Ensures on-going coordination of care between the comprehensive client, caregiver, physician, and other Comfort Keepers staff.

· Builds professional connections and relationships with other medical professionals such as physicians, directors of nursing in living communities, by introducing themselves and Comfort Keepers, sharing care plans, etc.

· Participate in on-call rotation. On call is by phone only.

Qualifications:

• Bachelor’s or Associates degree in nursing, BSN preferred
• At least 2 years of performing nurse duties
• Homecare experience preferred
• Current, unencumbered Minnesota license to practice as a Registered Nurse

Job Type: Full-time

Pay: $38.00 - $43.00 per hour
