As An LPN You Will
- Perform patient observation, data collection related to patient care, administers medication, treatment and procedures according to the physicians’ orders and resident care plan.
- Communicates patient findings to the supervisor.
- Accountability for facilitating, communicating and collaborating with the healthcare team to identify and meet the physical, emotional and spiritual needs of the patients.
- Oversee care provided by CNAs, Care Partners, QMAPs and/or other support staff, while also promoting the optimal health, well-being and safety of the patient through use of the nursing process and in accordance with patient care standards, guidelines and the State Nurse Practice Act.
- Demonstrate personal accountability for relationship-based care, organizational mission, and core values.
- One year of previous LPN experience, preferred
- Requires knowledge in nursing and person-centered care concepts
- Ability to demonstrate competency in the provision of individual care of patients based on the patients’ assessed age and developmental needs.
- HS GED
- Current state LPN license for applicable state of employment or current LPN license from a participating state in National Compact Licensure (Colorado only)
- BLS
This home health role will need to be open to cover any territory in the Lakewood.
