ResponsibilitiesThis position will be within Memorial Hospital 3 days per week and in the community 2 days per week
Collaborate with physicians, hospitals, and care facilities to provide coordinated and quality home-based services. Be the beacon of support and guidance in our healthcare landscape.
Assist discharge coordinators in coordinating home care services, empowering patients to make informed choices for their wellbeing.
Provide patients and families with compassionate education on home care regulations and services.

QualificationsState-Licensed RN
Nursing Experience, home health experience preferred
