• Position Summary*

Supporting Community Healthcare is a rewarding role. Mercy Medical Center Merced is looking for a Social Worker (MSW) to work in The Rural Health Clinics in our Street Medicine program.
The MSW works as part of an integrated multidisciplinary team including Community Navigators Medical Assistants Licensed Vocational Nurses Registered Nurses and primary care providers.
As a key member of this team the MSW is responsible for outreach and engagement mental health assessment and intakes care plan development short term case management collaboration with licensed mental health clinicians and the larger medical and mental health system.
The MSW works with the team and partner organizations to identify priority issues and needs to connect with medical and behavioral health care and to receive social support and resources needed to improve health and wellness.
Please note that this position is for our Street Medicine Program.
This position is a grant-funded position and may be eliminated if funds are no longer available.

• Qualifications*
• Minimum Qualifications:*
• One-year prior experience in a healthcare setting.
• Master’s Degree in Social Work, or other related field.
• Licensed Certified Social Worker OR must be a registered ASW with California Board of Behavioral Science.
• Must possess and maintain a current Basic Life Support (BLS) certificate.

Please be advised that this location requires quarterly American Heart Association (AHA) Resuscitation Quality Improvement (RQI) competency certification post-hire.
This ongoing competency is required through resources internal to the facility; outside RQI competency resources are not acceptable for CommonSpirit Health compliance requirements.

• Pay Range* $41.07 - $55.28/hour
