Community Care Center is Hiring a MDS/RN Near Seneca, MO
RN MDS CoordinatorSeneca House is currently looking to add an experienced MDS Coordinator to our team. As MDS Coordinator you will be responsible for preparing and electronically transmitting timely reports to Medicare and Medicaid. You will ensure accuracy with coding and documentation. You will assess and evaluate the care provided, Create care plans reflecting the personal choices and daily habits of each individual we care for. You will assist with the referral process by reviewing paperwork prior to admit to asses proper level of care and establish plan of care. Perform initial interviews and assessments, initiate and update plan of cares as required. Conduct meetings with individuals, multidisciplinary team members, caretakers and families. Rotating call schedule with nurse management teams.
